Shop policies, rules, and communication

  • Are there formal, written rules pertaining to shop access?

  • Are there formal, written rules pertaining to working alone?

  • Are there formal, written rules pertaining to working after-hours?

  • Have appropriate emergency response procedures been drafted and POSTED in the shop. At a minimum, emergency phone numbers and name/location of clinics and/or emergency responders must be posted.

  • Are there formal, written Standard Operating Procedures for high-risk activities such as working with exposed, energized electrical conductors, or control of hazardous energy during service and maitenance (i.e. Lockout/Tagout)?

  • Are there formal, written rules pertaining to the use of Personal Protective Equipment?

  • Have all applicable rules been communicated to shop users through postings, training, etc.? Has all training been adequately documented with date, names of participants, and an outline of the material covered?

  • Are all rules being consitently and effectively enforced? Are there effective consequences for non-compliance?

Personal Protective Equipment (PPE) and safe attire

  • Is eye/face protection provided and worn where needed?

  • Is hand/arm protection provided and worn where needed?

  • Is foot protection provided and worn where needed?

  • Is head protection provided and worn where needed?

  • Is hearing protection provided and worn where needed?

  • Is respiratory protection provided and worn where needed? Are all users of ANY TYPE of respirators enrolled in the University's respiratory protection program?

  • Other than PPE, are all shop users wearing safe attire (no torn or overly baggy clothing, no inappropriate footwear such as open toes, heels, flip-flops/sandals, etc.), no dangling jewelry, long hair is restrained or secured.

Electrical

  • Are all service panels and disconnecting means unobstructed (at least 36 inches in all directions)?

  • Is all electrical equipment in good condition and free of defects which could affect safety?

  • Is all electrical equipment approved/appropriate for the use at hand and being used in compliance with manufacturer instructions? No shop-fabricated equipment or tools?

  • Are electrical tools and equipment double insulated, effectively grounded, and/or protected by GFCI pigtails, receptacles, or breakers?

  • Are all flexible cords used appropriately (i.e. not used in lieu of permanent wiring, not secured or fastened to any portion of the building, not wrapped around objects, not run through floors, ceilings, walls, windows, not under floor covering, not run in series, etc.)?

Machines and equipment

  • Is all equipment approved/rated for the use at hand? Is there any shop-fabricated equipment?

  • Is all equipment properly guarded and free of defects?

  • Are all top-heavy machines secured to the floor to prevent tipping/walking?

  • Are compressed air nozzles equipped with appropriate venting, and/or is pressure regulated to less than 120 psi?

Fire prevention

  • Are all flammable/combustible material (including waste) properly stored/disposed of?

  • Is all spark-producing, open flame, and hot work conducted away from combustible materials?

  • Are there any signs of smoking? Are the University's smoke-free campus rules being observed?

  • Are there any electrical issues that may lead to fire?

Emergency preparedness and First aid

  • Is there adequate emergency egress from the area? Are all exits, and pathways to exits properly labeled, lit, and unobstructed?

  • Are emergency phone numbers and contacts posted near all phones? Is all information current and correct?

  • Is there adequate access to trained responders (within 4-6 minutes)?

  • Is there a means for summoning fire, police, etc. during all open hours?

  • Are there an adequate number of properly-rated fire extinguishers?

  • Are there appropriate first aid supplies?

  • If corrosive/irritating materials are in use, is there an appropriate (ANSI 358 compliant) emergency eyewash/shower? Is it being inspected/flushed at least weekly?

  • Is all emergency equipment and supplies provided in a location where they would be accessible at all hours that the shop is open (I.e. they are not kept in offices or other areas which may be locked at times).

Work environment

  • Are all slip, trip, and fall hazards controlled?

  • Is housekeeping and material storage neat and orderly?

  • Are all noise levels within acceptable limits (i.e. 85 dBA TWA)? Is hearing protection provided where appropriate?

  • Are there any exposures to airborne chemicals (dusts, mists, vapors, gases, fumes, odors) that should be evaluated to determine if they constitue a hazard?

  • Is the general work area and individual work areas properly ventilated (as needed)?

  • Is the work area maintained at appropriate temperature and humidity?

  • Are there any signs of infestation by mice, rodents, or other pests that might constitute a hazard?

  • Do employees have access to a private, sanitary toilet and handwashing facilities with soap?

  • Are all ergonomic and/or manual material handling practices adequately controlled?

Chemicals and hazardous waste

  • Are all hazardous chemical containers properly labeled with at least the trade name of the chemical (as referenced on the SDS), and a hazard warning?

  • Is there a current Safety Data Sheet (SDS) for each hazardous chemical in use?

  • Do all shop users understand the hazards of the chemicals in use? Do they understand chemical labeling and Safety Data Sheets (their purpose, where they are, how to understand them)?

  • Are all chemicals stored in a safe manner and properly segregated from incomatibles?

  • Is all Hazardous Waste stored and disposed of properly? Are all wastes clearly labeled with identity and accumulation dates? Are all wastes in closed/sealed containers? Are waste storage areas inspected weekly if not in the immediate work area?
