  • Is the machine driving in reverse or have a spotter when view is restricted?

Trenching / Excavations

  • Are there trenches or excavation on site?

  • Excavations 5' or greater shored, sloped or boxed?

  • Is excavated spoils stored at a safe distance from the excavation?

  • Is access / egress to excavations suitable?

  • Is equipment and materials kept at a safe distance away to prevent cave in?

  • Is water removed from excavations as soon as possible?

  • Are excavations being back filled as soon as possible?

  • Are ditches, and unsafe areas closed off with warning tape?

Material Storage

  • Is material stored neatly in stacks or piles and properly secured?

  • Is there adequate space between stacks of materials?

  • Fuel tank station positioned 25' from all activity, and 20' from building structures/ Port-a-Johns

  • Are fuel tanks double walled or in a suitable containment pit?

  • Is a properly sized fire extinguisher - Class 20B within 25' to 75', and is monthly inspection current?

  • Are tanks properly barricaded to prevent being hit? 5' clearance around fuel tank, and 12' wide path from gate to the tank?

  • Do fuel hoses have auto shut off?

  • Is there spill containment near by, or catch pan being used while fueling containers?

  • During fueling is the worker in constant control of the fuel hose?

  • Are tanks secured to prevent theft or contamination?

Chemical Storage / Compressed Gas

  • Are chemicals stored in an approved container with proper labels?

  • Eye wash stations accessible, current and provide 15 minute flow of tepid water?

  • Are gas lines, hoses and regulators in good condition?
