  • Is fall distance calculated, equipment selected appropriate to the distance and will deploy prior to impact?

  • Are anchor points supplied, installed and tested in accordance with AS 1891?

  • Are personnel working at heights using personal fall arrest systems 100% of the time?

  • Are only platform ladders with guardrails used on site?

  • Has the stability of the structure and the potential for collapse has been assessed?

  • Is there protection from falling objects in place (i.e. tool lanyards, exclusion zones, hoarding, catch nets)?

  • Is a rescue procedure in place with actions to be taken after an arrested fall has occurred?
