Pre-Operational Requirements

  • Can they locate inspection paperwork and perform pre-use inspection in accordance within company guidelines

  • Do they know "how to" report unsafe items in accordance within company guidelines?

  • Are they wearing required personal protective equipment (boots, hard hat, glasses, ear plugs)

  • Do they remove items out of any pockets that may get caught between harness and body if they suddenly fall into the harness?

  • Do they inspect the lanyard and attach it to rear d-ring before putting on the harness? (Attaching it after when you can't see it could result in the lanyard not hooking on at all)

  • Do they Inspect the harness for damage to the webbing, connections and d-ring? After putting on the harness do they ensure a snug fit?

Getting ready to use the lift

  • Are they able to explain the emergency lowering procedures?

  • Can they explain how they would cordon off or barricade an area to prevent someone from crossing under the lift?

  • Do they inspect the area in which the lift will operate to look for uneven or unstable surfaces?

Let's get into the lift and get started

  • Do they attach the fall protection lanyard to the basket before doing anything? Do they ensure the evaluator is also attached?

  • How do they do at explaining what each control in the basket does prior to doing anything?

  • Do they select the correct control and start to drive or steer? Are they able to drive the lift at a smooth, steady and safe speed? Are they watching for obstacles?

  • Do they set up correctly before attempting to raise the basket? Are they in an aisle or blocking a doorway? Can they point out all overhead hazards that may exist?

  • Are they able to maintain smooth steady operation of each stage while raising the basket? Keeping their eyes scanning to ensure enough space for the basket?

  • When asked to lower the basket do they check below for anything that may be in the way?

  • Are they lowering smoothly and safely? Are they lowering each stage of the boom at a time and correctly to avoid tipping?

Time to park it

  • Does the teammate put the basket all the way down? On a boom do they put the basket low enough to ensure a safe exit?

  • Does the teammate look to the rear before reversing the lift? Make sure there isn't anything or anyone in the path of travel?

  • Does the teammate park the lift in an area that is out of the way, ensuring the lift won't be hit by forklifts or other traffic?

  • Does the teammate turn around backwards and maintain 3 points of contact as they come down from the basket or deck?

  • Evaluator's comments (how did the teammate do? What do they need to work on?)

  • Trainee/Employee Signature
