Site Pre work, OH&S Coordination plan

  • For contracts over $350k Health and safety Coordination plan must be implemented..

  • Has nearest hospital and medical centre locations been identified?

  • Has emergency assembly point location been identified?

  • Has first aid kit locations and first aiders been identified?

  • Has site contact information been available to persons on site?

  • Has vehicle parking locations been identified or explained?

  • Has the process of loading/unloading delivery trucks or vehicles been explained outlining overhead power lines where necessary? (NO GO ZONES)

  • Has water supply locations been identified?

  • Have toilet locations been identified?

  • Emergency equipment available and serviced (fire extinguisher, spill kits, bunding)

  • Has rubbish bin locations been identified and general housekeeping explained to contractor or employee?

  • Traffic Management plan in place (as approved) as per plan and controlled? The TMP must always be available on site. Any changes must be documented. You cannot have less signs but you can have more

  • Is a site environmental management plan applicable to this site? if yes how has it been implemented...

  • Impacts on native vegetation, TPZ, wildlife habitat and Aboriginal heritage minimized, permits obtained and controlled?

  • Have all Risks & Hazards been discussed, identified and if so have controls been put in place and managed on site? and are they regularly reviewed?

  • is there any hazardous material onsite and if so are Safety data sheets available where required?

  • Have Safe Work Method Statements (SWMS) been understood, completed, signed off and for the works that are to be carried out onsite?

  • Does mobile plant or equipment operator have all relevant documentation such as Service history, operators manual, risk assessment, SWMS and complete a daily checklist. ?

  • Do your contractor (s) or employees have appropriate certificates or licensing to carry out works onsite? eg excavator ticket, electrical certifications etc...

  • Is there appropriate aftercare in place means of temporary fencing, bollards, barrier tape, parawebbing, wooden stakes, signage ?

  • Is appropriate PPE being worn by all onsite and appropriate PPE in use for activities? gloves, glasses, hi vis vest, hard hat, earmuffs and UV protection available,

  • Has power supply locations been identified?

  • Have all electrical tools and equipment been tested and have current tags?

  • Has Dial-before-you-dig and service locations been undertaken ?

  • Have underground services been identified and controlled? Gas (www.apa.com.au/globalassets/documents/miscellaneous/know-whats-below-brochure.pdf), Water, Sewer, Power, NBN ?

  • If trenching over 1.5m deep, have you notified WorkSafe? As you are legally bound to do so. (www.worksafe.vic.gov.au/construction-excavation-notification) if so please provide council with evidence

  • Have overhead electrical assets been identified and controlled and are outside NO GO ZONES? if inside NO GO ZONES a permit is required see www.worksafe.vic.gov.au/resources/using-powered-mobile-plant-near-overhead-assets-guidebook

Incident Notification

  • Are you aware of the process relating to reporting any incident including near misses, accident or injury ? if yes council must be notified withing 24 hours

  • Do you understand the procedures for reporting hazards? eg Asbestos, contaminated waste etc...

Sign off

  • Have you been given the opportunity to ask questions about anything on this form and has your question been answered so you have a full understanding of everything mention above?

  • Contractor to review and update safety documents including SWMS whenever they change in the scope of works or environment to be completed that is not outlined in my SWMS and add the task and make sure all onsite read, sign and understand?
