General

  • Are all employees familiar with the job to be performed and how to operate equipment safely?

  • Is workplace clean and in orderly condition (good housekeeping)?

  • Are all warning signs/barricades/labels/tags in place and legible?

  • Has a hazard assessment been completed or a pre-task evaluation been completed and documented to identify existing and potential hazards and how to protect employees from these hazards. (PPE, Barriers/Barricades, Fall Protection, etc...)

  • Do employees have required certifications and medical clearances on site?

Personal Protective Equipmentt

  • Are employees using fall protection, hearing protection, safety glasses, respirators, hard hats, safety shoes/boots, face shields, protective outer clothing for the job(s) being performed?

  • Has PPE been inspected by the employee prior to use?

Hazardous Materials

  • Are the SDSs available?

  • Are safety showers and eyewash stations within a reasonable distance from work being performed? If not, should have a portable eyewash at work location.

Ladders

  • If using an extension ladder, is it set up to a 4 to 1 ratio (for every 4 feet up, 1 foot away from structure)?

  • Are extension ladders properly secured at top (tied off) and extend at lease 3 feet beyond level of structure?

  • Are step ladders fully opened with employees not standing on the last 2 steps?
