  • Are all additional shores secured to prevent tipping ?

  • Have wood post been added under the trusses where the table leg lands in a opening or past the edge of slab ? (2 4X4s or 1 4X6)

  • Are tables built to match the Doka Assembly drawings ?

  • Are Tables installed the same as the sealed drawing ?

  • Have all the lashing straps and tie downs been installed and snuged on all edge tables ?

  • Have Doka beams been inspected for damage from flying previous pours ?

  • Are all the swivel head wedges tight ?

  • Are all the shores secured in the swivel head ?

  • Are all the shores the capacity shown on the sealed drawings ? (Eurex 20 - Eurex 30)

  • Are all the pins in the shores fully inserted ?

  • Have all the intermediate shores been installed and secured from tipping ?

  • Are all shores snug and plumb ?

  • Are all the pins in the shores fully inserted ?

  • Al-Shor RMD Tables

  • Is shoring installed the same as sealed drawing ?

  • Are all stringer and joist spans within the maximum span allowed shown on the sealed drawing ?

  • Is all cross bracing installed as shown on assembly drawings ?

  • Is the lateral bracing installed on all tables longer than 75' ?

  • Are there (6) M16 X 40 Bolts installed and tight at all Superslim stringers conections ?

  • Are all SS adapters attached to the Super Slim stringer with (2) 19MM pins ?

  • Are all the Al-shor Bases attached to SS adaptor with (2) 1-1/4" X 1/2" Bolts and nuts ?

  • Are all the pins in place top and bottom attaching the Al-shor to the base ?

  • Are all the ledger frames installed and locks in place ?

  • Does the screw jack extension meet the sealed drawing requirements ?

  • Are all screw jacks snug ?

  • Are all additional shores and stringers installed and secured to prevent tipping ?

  • Have all the tables been inspected for damage from flying previous pours ?

  • HI FLYER

  • Are the tables built to match the sealed drawing ?

  • Is any additional shoring installed the same as the sealed drawing ?

  • Are all the column hung jacks located acording to the drawing ?

  • Does the screw jack extension meet the sealed drawing requirments ?

  • Are all 1-1/4" threaded rods SEA Grade 8 or equivalent ?

  • Do all the jacks have two 1-1/4" dia. threaded rods conecting them to the columns ?

  • Are all the washers and nuts installed and tight on the 1-1/4" threaded rods ?

  • Are all the castelite beams properly seated in the screw jack head ?

  • Are all twelve 7/8" bolts at the castelite beam conections in place and tight ?

  • Are web stiffeners installed at all the jack locations ?

  • Are all the Transverse trusses attached to the castolite beams according to the drawing ?

  • Are there 7 on top cord and 7 on bottom cord 7/8" X 12" bolts installed and tight at all the transverse truss splices ?

  • Are there (2) A clamps conecting each joist to castolite truss ?

  • Have all the columns reached 100% of the specified 28 day strength ?

  • Have all the tables been inspected for damage from flying previous pours ?

  • J & S Column Hung System

  • Are tables built and installed the same as the sealed drawing ?

  • Has column concrete reached a minimum of 3000 PSI ?

  • Are the column shores and shims attached to columns or walls as per J&S drawing ?

  • Are all threaded rods and nuts the required 150 KSI 1-7/16" ? ( Lefthanded thread)

  • Are all nuts tight so that no gap can be seen between the shim, shore and concrete ?

  • Are the thrust washers in place between lift pad and adjustment screw ?

  • Are all the adjustment screws snug and at the proper elevation ?

  • Are all additional shores on the I beams in place per the sealed drawing ?

  • Is the center line of the I beam and center line of the adjusting screw aligned within +/- 1/2" ?

  • Are all assembly bolts in place and tight ?

  • Have the tables been inspected for damage to I beams and joist from previous pours ?

  • Hi Load Frames

  • Does scaffold layout match the sealed drawing ?

  • Is scaffold visually plumb ?

  • Are all scaffold braces installed and locking pins locked ?

  • Are all tie downs, lateral braces and lacing shown on the sealed drawing installed ?

  • Are hitch pins and rivets installed if required ?

  • Does the screw jack extension meet the sealed drawing requirement ?

  • Are all the stringers centered and secured in the J head or U head ?

  • Are baseplates supporting all scaffold ?

  • Doka Shoring

  • Is shoring installed the same as sealed drawing ?

  • Are all shores in place, snug, and plumb?

  • Are all the pins in the shores fully inserted ?

  • Is the shore spaceing within the maximum allowed shown on the sealed drawing ?

  • Are all the shores for special conditions installed ?

  • Are all the intermediate shores secured to prevent tipping ?

  • Has all braceing shown on the sealed drawing been installed ?

  • Al Shor

  • Is shoring installed the same as sealed drawing ?

  • Are all the ledger frames installed and locks in place ?

  • Are AL-Shor Pins in place ? (if required)

  • Are all screw jacks snug ?

  • Does the screw jack extension meet the sealed drawing requirements ?

  • Are the AL-Shor frames installed on a sloping surface ?

  • Is the slope less than the maximum allowable slope ? (5 deg)

  • Have braces been installed to nurtalize lateral loads ?

  • ULMA System ( ENKOFLEX and ALUPROP )

  • Is shoring layout installed the same as sealed drawing ?

  • Are all Aluprop shores in place and snug ?

  • Are all shores visually plumb ?

  • Are all prop joint connectors in place and installed properly ? (if required)

  • Are all the bracing frames installed and at the proper height and spacing ?

  • Do all shores have the proper lap with the top a minimum of 2" below the clamp ?

  • Are all the shores in place, snug, and plumb?

  • Do all the shores have 2 nails above the clamps ?

  • Have the extra shores been added under beams and in all the required areas ?

  • Have all shores been checkes for cracks, splits and bows ?

  • Is all X bracing in place at the pour bulkhead and building perimeter ?

  • Is any lacing required ?

  • Is lacing installed at the location and in the direction shown on the sealed drawing ?

  • Is the lacing nailed securely to all the Ellis Shores ?

  • Hybrid System

  • Has all additional shoring been installed as shown on the sealed drawing ? (If required)

  • Are all truss the feet fully bearing on beam ledge?

  • Are all the slider wedges tight ?

  • Have all the trusses been inspected for bent or broken webbing ?

  • Are all braces installed on the pre-cast column or beams ? (If required)

  • Are all the erection bolts loose ?

  • Have the offsets in the table tops been corrected ?

  • Have all filler plates been secured and chaired down ?

  • Are the beams across the opening the same size and material shown on the sealed drawing ?

  • Does the spacing match the sealed drawing ?

  • Is this pour on a ramp ?

  • Do all shores have wedges installed to give full bearing on slab below ?

  • Do stringers and joist have wedges installed to provide full bearing ?

  • Are any shores on mud-sills ?

  • Does the mud-sill design match the sealed drawing for size and material used ?

  • Are all the mud-sills centered under shore leg and bearing completely on dirt or gravel ?

  • Is compaction and bearing enough for the leg load ?

  • Are all mud-sills in good sound condition ?

  • Have efforts been made to prevent runoff from washing under mud-pads during heavy rains ?

Stringers and Joist

  • Stringers

  • Are the stringers used the type shown on the sealed drawing ?

  • Are stringer installed in the same direction shown on the sealed drawing ?

  • Does the stringer spacing and spans match the sealed drawing ?

  • Are all the stringers centered in the heads ?

  • Are the stringers properly secured to the shoring ?

  • Are all cantilevers equal to or less than the maximum allowed on the sealed drawings ?

  • Joist

  • Are the joist used the type shown on the sealed drawing ?

  • Does the joist spaceing and spans match the sealed drawing ?

  • Are all cantilevers equal to or less than the maximum allowed on the sealed drawings ?

RESHORES

  • Do you have a Sealed Reshore drawing on site for this pour ?

  • Does the reshore grid match the sealed drawing ?

  • Is the reshore required leg to leg ?

  • Have you verified that the reshore is located leg to leg ?

  • Do the shores used have the rated capacity listed on the sealed reshore drawing ?

  • Are all the shores snug and plumb ?

  • Have spring clips been used on all the post shores ?

  • Are the correct number of reshore levels installed ?

  • Have the lower level reshores been rechecked ?

  • Have all the levels supporting formwork reached required strength ?

  • Are the perimeter reshores secured to prevent falling from the building ?

  • Are any reshores on mudsills ?

  • Does the mud-sill design match the sealed drawing for size and material used ?

  • Are all the mud-sills centered under shore leg and bearing completely on dirt or gravel ?

  • Is compaction and bearing enough for the leg load ?

  • Are all mud-sills in good sound condition ?

  • Have efforts been made to prevent runoff from washing under mud-pads during heavy rains ?

  • Are all outriggers reshored for (3) floors ?

  • Steel Post Shores

  • Are all the pins in the shores completely inserted ?

  • Do all Ellis Shores have 2 nails above the clamps ?

  • Have the Ellis Shores been inspected for splits, cracks and bows ?

  • Is any lacing required ?

  • Is lacing installed at the location and in the direction shown on the sealed drawing ?

  • Is the lacing nailed securely to all the Ellis Shores ?

  • Scaffold

  • Are all the towers located as shown on the sealed reshore drawing ?

  • Are all scaffold braces installed ?

  • Do all heads have stable blocking between the head and concrete ?

  • Does the extension on the screws meet maximum extension requirments ?

  • Shoring left in for reshore

  • Was this shoring left in place because of concrete strength or PT issues ?

  • Has a new reshore plan been designed for this condition ?

  • Has a new sealed drawing been done for this condition ?

  • Has the reshore been installed exactly by the new sealed drawing ?

  • Have all the additional reshore levels been installed and inspected ?

  • Has this new reshore design been inspected by a qualafied person ?

  • Has the shoring load been released and re snuged ?

  • Is all shoring includeing any added shores still in place ?
