General Health and Safety Questions

  • Are workers trained and competent for the task and do they understand the risks (includes working at heights, rope access, using EWPs, and erecting and using scaffolding)?

  • Is the work group adequately supervised for the task (e.g. have supervisors visited the worksite, enough supervision for the workgroup and task complexity, and how much experience the supervisors have in the role)?

  • Has the work group completed a risk assessment, and do they have the correct instructions / procedures for the task?

Specific Risk Questions

  • Are workers exposed to a fall risk without controls in place (includes open edges, bypassing guarding / handrails, and at height on vehicles and mobile equipment)? Provide information on controls used.

  • Are workers using the correct Working at Heights PPE for the task?

  • Are workers using barriers, tethers and storage to prevent tools and objects from falling?

  • Are barriers in place to separate workers from areas exposed to dropped objects, tools, and materials?

  • Are EWPs / Scissor lifts fit for purpose for the task (meets inspection requirements, no significant faults, and meets maintenance requirements)?

  • Are workers using fit for purpose steps, ladders and platforms (where applicable)?

  • Is equipment (including mobile equipment) inspected and approved to be onsite?

  • Are workers aware of the emergency response processes should an incident occur (e.g. what emergency response preparations need to be in place before commencing work, the process and details to contact emergency response, and what immediate emergency action can be taken)?

  • Is there an opportunity to eliminate or reduce the hazard by implementing an Identify Then Rectify (ITR) safety improvement?
