Is the MEWP for your own use? If no, hire must be declined, only the colleague using the MEWP can request hire.

Have you undertaken training and obtained an IPAF card or other pass certificate within the last 5years on the use of MEWP's? <br>If no, hire must be declined.

Have you planned how the job is to be carried out using the MEWP?<br>If no then this must be undertaken by the colleague requesting hire of MEWP before hire is authorized.

Have you read, understood and confirm you will work to the risk assessment CFMRA 19 MEWPs?<br>If no, then this must be read by colleague(s) requesting hire of MEWP before hire is authorised

Do you have a safety harness with suitable lanyard length available to use with the MEWP?<br>If no, hire must be declined the colleague will require to have a safety harness.

Has the safety harness been checked in the past 3 months and a record kept of the inspection on form CFMHS32? If no, hire must be declined the colleague will require to have a safety harness checked with the last 6 months

Will you be using safety barriers to exclude other persons from a minimum of 1 mtr from the circumference of the MEWP although ideally 2 mtrs where possible?<br>If no, hire must be declined unless the colleague is able to obtain safety barriers with the use of the MEWP.

Are you aware that a Weekly MEWP inspection must be carried out and recorded on form CFM HS 31 before first use and on every 7th day of hire? If no, hire must be declined.

You must have a spill kit available to deal with diesel or hydraulic oil leaks. The store spill kit if available can be utilised when work is internal and away from drainage systems. If works are external or near drainage (within 10m) a spill kit ,must be included with the hire. Tick box is spill kit is required to be hired with MEWP
