Wire mesh and debris tray netting are intact and free of debris

Emergency decent function is operational

Crew members are properly trained

Crew in swing is equipped with a radio for communication with groundman

Retractables, lifelines, and rope grabs are in good condition with out defects

Are secured to structure or engineered anchorage point

Edge protection is implemented on all sharp edges

Lifelines are tied with a bowline knot or termination bar

Guardrails, warning lines, or PFAS are in place at all leading edges

All tools are tethered and secured to worker or basket

All materials are tethered before they are permanently fastened to the building

All small loose items are stored & secured within a zipped canvas bag and tethered

All materials/tools are tethered before they are transferred between the roof and swing basket

CAZ is established around affected area

CAZ consists of hard barriers (NO CAUTION TAPE)

Appropriate signage is posted

Groundman is present

Groundman is equipped with a radio for communication with swing crew

Daily suspended scaffold log book is completed

Swing specific PTP has been conducted, reviewed, and signed off by crew
