Covid-19 Screening

1. Do you currently have or have you experienced any of the following symptoms during the past 48 hours or since your last shift? Cough, shortness of breath, headache, fatigue, fever (100.4* or higher), muscle or body aches, sore throat, loss of taste or smell, nausea or vomiting, diarrhea, congestion or runny nose

2. Do you currently live with someone who has a laboratory confirmed case of Covid-19? (if crew member answers YES, they should be sent home immediately and should follow local health official or guidance for quarantine).

3. Have you been told to quarantine or isolate due to potential exposure or close contact with Covid-19? (if crew member answers YES, they should be sent home immediately and should follow local health official or guidance for quarantine).

4. Have you recently been tested for Covid-19 and are awaiting results? ( if the crew member answers YES, they should be sent home immediately until results are received. The individual may return to work if test results are negative and they have no symptoms and have completed any required quarantine).

If the crew member is experiencing any of the symptoms, CDC and state guidance recommends they must remain home for at least 10 days from onset of symptoms AND show improvement in symptoms (e.g., cough, shortness of breath) AND is fever-free without medication for at least 27-72 hours).
Uniform Screening

1. Pants: black slacks must not be jean, yoga or leggings allowed (e.g. black slacks with no sewn back pockets), slacks must have a belt, Capri's may be worn (girls only).

2. Name tags: All crew members that come in visual contact with our guests must wear Sonic issued name tags. Name tags must be worn on the opposite side of the Sonic logo.
Appearance

1. All personnel must be clean shaven with the exception of a neatly trimmed mustache.

2. No excessive makeup, perfume or cologne is allowed

3. Fingernails must be clean, be of normal length (i.e., no to extend further than 1/4 inch from finger tips), not have nail polish (color or clear) and not have tips or add-ons.

4. No visible facial or body piercings are permitted withing the customers view

5. Visible tattoos must not be offensive and may not include references to profanity, gang association, racial or sexual content.

6. Hair is to be clean, well-groomed and neat. If hair length is below collar of shirt, it should be pulled back and contained at all times ( restraint in a bun or pony tail). A hair net may also be used as an option.

7. Shirts must be clean and not torn or have holes. Must be pressed and tucked in before arriving at the drive-in.

8. Aprons must be clean and worn properly.

9. Shoes must be enclosed (no sandals) , slip resistant and in good condition (no holes), black in color.

10. Cell phones, MP3 players, PDA's and similar electronic devices and cameras may NOT be carried, worn or used at work. Managers may carry cell phones in case of a business emergency.
