  • 1. Is there fixed edge protection in place?

  • 1.1 has it been designed by an Engineer, with experience in compliant edge protection?

  • 1.2 has it been errected in full compliance with the design by a Rigger or Scaffolder?

  • 1.3 does it have a handrail (mimimum hieght of 900mm, mid-rail, saced at maximum 450mm and a toe-board mimimum height of 100mm)?

  • 1.4 where steel mesh panels are used instead of guardrails is the mesh at least 4mm and an aperture no larger than 50mm x 50mm with a kick plate at least 150mm high?

  • 1.5 is there a scaff tag in place on or near where the edge protection has been installed?

  • 2. Is there a risk of falling objects?

  • 2.1 is there a screen in place to manage the risk of falling objects (gaps in the screen must not exceed 25mm, been installed by an Advanced rigger or Advanced Scaffoler and is there a handover certificate)?

  • 2.2 are all hand held tools secured and tethered?

  • 2.3 has an exclusion zone using physical barriers been established?

  • 2.4 is there a sign in place warning workers who may attempt to enter the exclusion zone, not to enter?

  • 3. Is there scaffold on-site?

  • 3.1 has the scaffold been constructed using mixed components? NOTE: this is not permited

  • 3.2 is there a scaffold plan (prepared by an Advanced Scaffolder)?

  • 3.3 has the scaffold been erected as per the plan?

  • 3.4 if the scaffold is higher than 4m was it erected by a ticketed scaffolder?

  • 3.5 is there a handover certificate for the scaffold?

  • 3.6 is there evidence of inspection (scafftag) at the entry points?

  • 3.7 has an inspection been conducted within the last 30 days?

  • 3.8 are the scaffolds physically protected from being struck by mobile plant, light vehicles etc?

  • 3.9 are the castors on mobile scaffold locked?

  • 4. Are there Mobile Elevated Work Platforms?

  • 4.1 are all boom type EWP's fitted with scondary protection systems?

  • 4.2 is the log book and manual available in the machine?

  • 4.3 has the operator conducted and documented a pre-start inspection?

  • 4.4 do operators of boom lifts with a boom length greater than 11m hold a relevant high risk work licence?

  • 4.5 are the MEWPs regularly inspected including Pre-Start, Routine, Annual and Major Inspections and are the inspections recorded in the logbook?

  • 4.6 do scissor lifts have a guard located at lest 50mm above the highest point of the controls to prevent unintentional activiation?

  • 4.7 is the emergency personnel retrieval procedure, appropriate to the operation and task is clearly outlined in the SWMS? The retrieval procedure may be as simple as the nominated ground person(s) lowering the basket of the machine using the ground controls.

  • 4.8 is the MEWP positioned to enable the ground controls of the MEWP to be accessible by ground personnel at all times?

  • 4.9 is/are the nominated ground person/s line of sight with the operator(s) at all times?

  • 4.10 is the means of communication between the operator(s) and the nominated ground person(s) outlined in the SWMS and in place?

  • 4.11 is there evidence to demonstrate that ground personnel have been trained in the use of the emergency retrieval systems and did the training include a practical assessment i.e. the MEWP operator extending the MEWP via the ground controls and having the ground person demonstrate that they can lower the machine using the ground controls?

  • 5. Are there penetrations and voids?

  • 5.1 are horizontal or vertical penetrations covered with material of adequate strenght to prevent objects falling through them?

  • 5.2 are all covers set flush ot have bevelled edges so a trip hazard is not cretaed and must be clearly marked with the words 'Warning - Hole Below'?

  • 5.3 if the penetration cannot be covered, has the area below been designated as an exclusion zone?

  • 6. Are platform ladders being used?

  • 6.1 do platform ladders 2 meters of higher have a drop bar or back bar?

  • 6.2 is each platform ladder subject to a three monthly inspection?

  • 7. Is fall prevention equipment being used?

  • 7.1 have the personnel wearing the equipment been trainined in the use of the equipment?

  • 7.2 are personns attached to a certified anchor point?

  • 7.3 do the anchor point have a tag indicating the SWL?

  • 7.4 have drill in anchor points been proof tested?

  • 7.5 are harnesses fitted with suspension trauma straps?

  • 7.6 do the carabiners have double locking gates and the load capacit and manufacturers name clearly marked?

  • 8 Emergency planning

  • 8.1 is there a rescue plan outlined for activities involving working at heights? (could be outlined in the SWMS)

  • 8.2 does the plan outline the equipment required for rescue, its location and the communication mechanisms to be adopted?

  • 8.3 have workers been briefed in the resuce method?

  • 8.4 is the equipment close by?
