OBSERVER PLANNING NOTES

  • 1) Notify respective supervisor of audit in area.
    2) Introduce yourself to worker being observed.
    3) Review the observation checklist with the worker.
    4) Stand in a safe place to complete observations.
    5) Complete feedback session (start with Safe items)
    6) Submit completed form for data collection and review

ERGONOMIC ACTIVITIES

  • Is the upper body, shoulders, and neck in neutral position?

  • Are wrists kept in neutral position when holding tools?

  • Are knees bent and head upright when lifting?

  • Are elbows at sides and loads held close to body when lifting?

  • Is lifting motion smooth (avoids jerking)?

  • Do hands and arms stay below shoulder height when lifting or reaching?

  • Does the worker pivot feet when turning and carrying a load (upper body not twisting at the waist)?

  • Is there a mechanical lifting device used to position the load/work?

  • Are carts, pallet jacks, etc., pulled instead of pushed?

WORK RELATED ACTIVITIES

  • Is the worker wearing safety glasses with side shields?

  • Is filtered eye protection worn when tack-welding with electric arc welders?

  • Is the immediate work area clean and free of tripping and slipping hazards?

  • Are gloves worn when handling sharp edged material?<br>(expanded metal, sheet metal, steel strapping)

  • Is forearm protection pulled up when worn?

  • Are machine guards in place and not bypassed?<br>* Gears are not touchable<br>* Belts and pulleys art not touchable<br>* Roller chain and sprockets are not touchable<br>* Power transmission couplings are not touchable

  • Is work material stacked so that it doesn't fall over?
