Life Safety

  • Are all exit ways, doors, and aisles clear and unobstructed?

  • Are exit signs lit and visible from all parts of the plant area?

  • Are emergency escape routes marked, lighted and clear?

  • Is there an emergency exit route plan by every exit doorway?

  • Have all employees been trained in the sites emergency action plan?

Occupational Health and Safety

  • Proper gloves being used for hand protection?

  • Are dust masks being worn when needed?

  • If respirators are required have all personnel had a PFT, fit test, and medically cleared?

  • Are all employees carrying a company supplied I.D. card?

Temporary Employees

  • Are all temporary employees easily recognized? (i.e. different colored hard hat)

  • Have all temporary employees been through the Company Orientation process?

  • Are all temporary employees being mentored?

  • Have all temporary employees been issued required PPE from the staffing agency?

Fire Protection

  • Are "No Smoking" policies being enforced?

  • Are fire extinguishers being inspected at least monthly?

  • Are all fire extinguishers properly marked and identified as to their location?

  • Are all discharged fire extinguishers taken out of service and replaced with full ones?

  • Are all discharged fire extinguishers documented and sent to the tool room for replacement?

  • Are "Hot Work Permits" being used when necessary?

  • Are fire watches being used for at 30 minutes after the Hot Work has been completed?

Machine Guarding

  • Are all pinch points guarded?

  • Are all employees properly trained in machine guarding?

  • Are all gears, chains, and pulleys, and conveyors properly guarded?

  • If guards are not in place have the machines been deenergized and locked out per the LO/TO policy?

Flammable and Combustible Materials

  • Is all combustibles being properly stored in covered fire proof containers or promptly removed from site?

  • Are all flammable and combustibles being properly stored in a flammable storage cabinet?

  • Are fire extinguishers more than 35' but less than 100' from all flammable and combustibles?

Compressed Gas Cylinders

  • Are all cylinder clearly marked as to their contents?

  • Are oxygen and acetylene bottles stored at least 20 feet apart or separated by a 5foot tall 30 minute burn wall?

  • Are all cylinders stored in an upright position?

  • Are all cylinders stored away from excessive heat?

  • Are empty cylinders labeled as Empty?

  • Are empty and full cylinders stored away from each other?

  • Are protective caps in place when cylinders are not in use?

Power Tools and Equipment

  • Do rotating tools have protective guards in place if supplied by manufacture?

  • Are all electric tools grounded or double insulated?

  • Were tools inspected prior to use?

  • Are tools in good working order?

Housekeeping and hygiene

  • Are all slip, trip, hazards picked up?

  • Is the work are kept clean throughout the shift?

  • Are all spills cleaned up and wet floor signs used as necessary?

  • Are restrooms kept clean, sanitary and properly stocked?

  • Are breakrooms kept clean?

  • Refrigerator is emptied at the end of every shift?

Compressed Air and Compressors

  • Are hoses properly rated for the PSI?

  • Are hoses in good shapes with no cracks or flat spots?

  • Hoses are being properly maintained?

  • Hoses are still flexible?

  • Hoses are secured with a pin in the coupler and whip checks in place?

Electrical

  • Are all electrical panels properly labeled?

  • Are all flexible cords in good shape?

  • Ground prongs in place where needed?

  • No cuts in the insulation?

  • No splices?

  • Cords are properly rated for amperage, load and length?

  • Cords have not been pulled from the end plugs making the inside insulation visible?

Walking Working Surfaces

  • Surfaces are kept without trip hazards?

  • Are stairways are in good working order with proper handrails in place?

Lighting

  • Is work area properly lit?

  • Do any bulbs need to be replaced?

  • Do light fixtures have cages covering the bulbs?

Forklifts and Lifting Equipment

  • Are operators trained and certified on the lift they are operating?

  • Do operators have their certification card on them?

  • Are forklifts being operated in a safe manner?

  • Are forklifts being operated at a safe speed?

  • Is proper PPE being used when applicable?

  • Is the forklift being inspected for mechanical defects before every shift?

  • Are forklifts being maintained in a safe manner?

  • Are all straps, slings in good condition, no cording showing or wire rope broken?

  • Are all chains in good shape with no damaged links?
