Vehicle Paperwork

  • Current copy of proof of insurance in the truck

  • Current DOT Annual Inspection present on the truck/trailer

  • Current copy of the registration present on the truck/trailer

  • Copy of CabCard specific to the unit with VIN Number present in the truck and jurisdictions in which it is operating in.

  • The last DVIR matches the odometer/hub odometer onthe truck.

Safety Equipment

  • A properly mounted, charged, and inspected 5 BC or greater fire extinguisher is present in the truck

  • A first aid kit is present in the truck.

  • Accident reporting kit present in the truck

  • Three reflective triangles are present in the truck

  • Spare bulbs, fuses, and headlamps present on the truck (If Applicable).

Equipment (Truck/Trailer)

  • The truck has DOT numbers, EES logo, and spelled out name as listed on MCS-150 form on both sides of unit and visible at 50 feet

  • All lights (i.e. Brake lights, tail lights, turn signals, headlamps, license plate, etc.)work on truck/trailer, and all lenses are in place.

  • Tires and Wheels - Check tires for wear, wheels for cracks, missing lug nut and bolts, valvestems, and metal valve stem caps.

  • All chains on truck/trailer haveno apparent stretch in links, and hook on both ends of chains properly secured to chain.

  • All tie down straps on the truck/trailer, used to secure load have not cuts or gashes.

  • For trailer - Red and White reflective tape half the length of the bedof the trailer and the width of the rearof the trailer, present in good order.

  • All loose debris or equipment in the cab of the unit thatcould become projectiles in the event of a severe braking action or vehicle accident is removed.

  • No visible drips or leaksfrom any hoses in or under the unit.

  • Suspension-spring handers, leaf springs, torsion springs

  • Axels free from cracks or obstructions

  • Horn - Air or electric

  • Windshield/ wipers/ fluids

  • Air leaks - Audible leaks at proper connections and in lines

  • Chaffing air hose/lines - Lines cannot rub against frames and may need sheath cover.

  • Coupling devices - 5th wheel locked, bolts tight, no cracks in upper or lower devices. Safety Chains (2) cradled. Safety Hooks must have latches.
