All workers in your team and work zone must be consulted on how you will manage the identified hazards. If you do not have one prepared for this site there are templates linked to the trades types/task section that you may use to make your own.

I have read the applicable SWMS all of my team have been inducted into the applicable SWMS for the activities to be conducted, I have engaged other trades/persons on site and reviewed each others activities and taken that into account as part of completed the following hazard assessment.

Site/Job Supervisor SWMS review sign off

The site/job supervisor should review the SWMS when available, for the purposes of identifying hazards that may be present on site that are not within the reasonable skill set of the contractor completing the swms for the task. If the Supervisor is not on site please, check the builders sign or site information notice board for specific site hazards, if unsure call to check for any specific hazards.
