1.1 - Is the forklift safe to operate?<br>Note: Report any unsafe condition immediately and do not operate.

1.2 - Did you report the unsafe condition of the lift?

1.3 - Did you remove the key and place a lock-out / tag-out sign on the lift's steering wheel?

1.4 - Did you forward the key to a supervisor, manager, or maintenance personnel?

1.5 - Is there any visible damage that was not reported from the previous shift?

Note: Take a picture and report any unsafe condition immediately. Do not operate the forklift.
Section 2.0 The Identity of the Forklift

2.1 - Is the Operator Manual available?<br>Note: Check under the seat or in storage bins.

2.2 - Can you read the Name Plate and is the model number the same as listed on the manual?
-
3.0 - Are the warning decals legible?
3.0 - General

3.1 - Forklift body is free of excessive greases, oil, or filings.
-
3.2 - Is the Overhead Guard secured to the body of the lift?
-
3.3 - Is the Mast Guard (behind the forks) cracked, have hole(s), bent, or damaged?

3.4 Does the Seat Belt work properly?
-
3.5 - Is the Overhead Guard cracked, have hole(s), bent or damaged?
-
3.6 - Turn the Key to the ON position but Do Not Start. Are dash board instruments working properly?

Note: Take pictures of any general forklift hazards
4.0 - Tires
-
4.1 - Are the tires in bad shape, have large holes, tearing, long deep cuts, or have rubber missing near the rim?
-
4.2 - Are wheel nuts on the rim, tightened, and not damaged?
-
4.3 - Does the lift have the correct tires as specified in the manual and surface condition?
-
Note: Take pictures of any tire or wheel hazards
5.0 - Under the Hood
-
5.1 - Does the hood latch work or fasten properly?
-
5.2 Do you see any visible leaks on the side or draining from beneath the lift?
-
5.3 Did you check the engine oil dipstick for the proper level?
-
5.4 Did you check the Hydraulic Fluid reservoir (tank) for the proper amount?
-
5.5 - Did you check the Transmission Fluid dipstick for the proper amount?
-
5.6 - Did you check the Hydraulic Fluid reservoir (tank) for the proper amount?

Note: Take pictures of any Under Hood hazards
Section 6.0 - Hoisting Attachments (The Forks and Mast)

6.1 - Are forks cracked, bent, worn, or curled at the end?
-
6.2 - Does the Mast or Hoisting Frame have cracks, broken welds, bent, missing hardware (stops), missing bolts.
-
6.3 - Are the lift chains greased, secured, and free of kinks?

Section 7.0 - Hoses

7.1 - Are the hydraulic lines leaking?

7.2 Are hydraulic joints leaking?

7.3 - Are the hydraulic lines worn and show metal wires?

Operational Inspection
General Safety Devices.
-
All lights and turn signals are aimed correctly, and are working properly.
-
Horn works.
-
Gauges are working and reading within proper margins.
-
Backup alarm is sounding when in reverse.
Drive Controls
-
Accelerator is working properly and functioning smoothly
-
Drive control (forward and reverse) functioning smoothly.
-
Steering is smooth, responsible, and free of excess play.
-
Service brake is functioning properly and smoothly and does not easily drop to the floor, or grab. There are no fluid leaks.
-
Parking brake is working properly.
-
Engine is running smooth without leak or sparks from the exhaust.
Lift Controls
-
Lift and lower controls operate smoothly and without squeaking or excessive drift.
-
Tilt control operates properly without excessive drift, chatter, or squeaking.
