First Impressions - Engineer & Appearance

  • Is vehicle parked correctly and at a suitable location?

  • Is vehicle clean outside?

  • Is vehicle tidy inside?

  • First Aid kit on vehicle?

  • Is it within expiry date?

  • Engineer must replace items or kit and ensure adequate First Aid kit is carried on vehicle

Working at Heights - Roof work

  • Was Roof Access required?

  • Pitched roof or Flat roof?

  • Does the flat roof have edge protection?

  • Was the engineer safe and risk of falls minimised?

  • Was a 2m exclusion zone set up around the roof edge?

  • Were suitable Anchor points available?

  • Was a man anchor required? (Dead mans anchor)

  • Was the man anchor set up correctly with full fall restraint system in place?

  • Eyebolts drilled/Anchor points used and fall restraint system set up correctly?

  • Was Roof ladder set up correctly? (Strapped to triples etc)

Manual Handling

  • Have mechanical means been used?

  • Are safe lifting techniques being observed?

Electricity

  • Has electrical isolation taken place?

Equipment

  • Is electrical equipment in good condition?

  • Is any work equipment safe and free from wear and tear??

  • State which pieces and remove from service

Personal Protective Equipment (PPE)

  • Eye protection used when drilling?

  • Ear protection used? (Defenders or plugs)

  • Hand protection worn when required?

  • Dust masks worn when required?

  • Overshoes worn? (If applicable)

  • Where PPE is not required is it stored appropriately?

  • Explain to engineer the importance of correct storage of PPE

Signage & Barriers

  • Were road or pathway cordons set up?

  • Should road or pathway cordons have been implemented to protect the public etc?

  • Details and corrective actions taken place

  • Photos of areas that should have been cordoned off

  • Correct use of Cones?

  • Correct use of Barrier tape?

  • Correct use of signage? (Cable in progress etc)

Asbestos

  • Has asbestos been identified on site?

  • Have asbestos precautions been followed?

  • Work must be stopped immediately, H&S department and line manager contacted and the asbestos register consulted.

Vibration

  • Is risk from exposure from vibration being adequately controlled?

Lighting

  • Is lighting suitable for task?

  • Ensure task has sufficient lighting before continuing

Completion

  • Has a full H&S kit inspection been carried out within the last 6 months?

  • A full Health & Safety kit inspection is to be carried out by a competent person. H & S department to be contacted
