Section 1 - General Safety

  • 1. Are walkways and doors clear from obstructions?

  • 2. Are floors and carpets in good condition?

  • 3. Is lighting steady with no blown bulbs?

  • 4. Are items stored above ground secure from falling?

  • 5. Are electrical plugs and cords in good condition?

  • 6. Are waste bins provided and regularly emptied?

  • 7. Are Safety Data Sheets available for chemicals?

  • 8. Are First Aid and Fire Warden identities displayed?

  • 9. Do employees know Fire Wardens/First Aiders? (Ask)

  • 10. Are evacuation posters displayed?

  • 11. Are First Aid Kits available with updated supplies?

  • 12. Are Safe Work Procedures available/known? (Ask)

  • 13. Are WHS Committee minutes available for staff?

Section 2 - Office Safety (Use a preferred sample of at least 20% of workers in the area assessed)

  • 1. Are standard workstation items within easy reach?

  • 2. Is the top of the PC monitor at eye level?

  • 3. Are elbows and knees at 90 degrees when seated?

  • 4. Are headset cleaning items available and used? (Ask)

Section 3a - Warehouse Safety

  • 1. Are pallets stored above the 2nd tier wrapped?

  • 2. Are forklift checks completed before each shift?

  • 3. Is there evidence of Traffic Management controls?

  • 4. Are forklift keys removed when not in use?

  • 5. Is PPE available and used? (eg Vests, Shoes, Gloves)

Section 3b - Safety Behaviours (Optional - Take 15 minutes to view the work activities for behavioural safety in the workplace)

  • 1. Manual Handling - Correct methods used?

  • 2. Forklifts - Seat belts/speed limits/traffic controls?

  • 3. Pedestrians - Following traffic controls as per plan?

  • General Comments (Optional. Comments on any identified concerns not noted above).
