1-Job Planning and safe systems of work description.

  • Has the Inspector undertaken a JSA assessment of the risks involved prior to carrying out the work?

  • Is the JSA completed accurately,Dates,Address,Customer Name and Work Order.

  • Has the contractor written the job steps and considered the hazards of each step?

  • Have all persons on site signed the JSA. Can they demonstrate an understanding of JSA's

  • Have all persons read and understood the Safe Work Method Statement ?

  • Are all the Inspectors carrying their WHITE CARD.

2-Prevention of Falls.

  • Is it possible the Inspector could fall thru ceiling?

  • Does the ceiling contain brittle or fragile materials in the work area or traffic routes?

  • Has the Inspector taken steps to identify and control environmental factors,which may increase the likelihood of falls ,Moisture,Dust,Heat,Moss

3-Electrical Safety.

  • Is the inspector aware of nearby electrical services and is providing a safe system of work?

  • Has the Inspector identified , isolated and tagged the Switchboard? Correct number of tags on switchboard.

  • Are all electrical supplies including Solar isolated?

  • Did the inspector check for live cables upon entering roof space?

  • Is there foil insulation in the roof space?

  • If yes has the inspection been stopped?

4-Access and Egress

  • Are ladders used for access?

  • Does the ladder have non slip feet?

  • Is there a safe landing place when stepping off the ladder?

  • Has the ladder been tested and tagged current for this month?

  • Does the ladder have an asset number on it?

  • Is there a drop sheet in place?

5-Tools and Equipment.

  • Do the tools and equipment appear to be in good order?

  • Is the footware appropriate for working in the ceiling?

  • Is the Inspector using safe methods to take tools and equipment to work area?

6-PPE

  • Does the inspector have all the PPE with him?

  • Is the Inspector wearing the full correct PPE for the job Being done?

7-Work Area

  • Are materials and preparation areas orderly,clean and tidy? ( Trip hazards have been minimised )

  • Are the correct clean up procedures being followed?

  • Has the inspector taken enough steps to prevent persons entering the work area below the manhole or external work zone? Signs , Witches Hats or hazard tape.

8-Training and Supervision.

  • Can the Inspector correctly perform the work ?

  • Can the Inspector access and exit the ceiling and work area safely?

  • Can the inspector demonstrate that all persons carrying out the work have received appropriate training and instruction?

  • Can the Inspector demonstrate the correct fitting of all safety and PPE.

9-Emergency Preparedness

  • Can the Inspectors implement the controls noted on the JSA's?

  • Can the Inspector demonstrate a process for responding to an emergency.

  • Can the Inspectors staff demonstrate an understanding of the emergency responsive,what is their role in an emergency?

  • Does the Inspector have a First aid Kit? Are the contents up to date?

  • Is the vehicle carrying a Fire Extinguisher. Fully charged.

  • Does the contractor have a copy of Emergency phone numbers.

  • Does the Inspector have copies of the Material Safety Data Sheets?

  • Does the inspector have copy of the Van Pack with him?

  • Does the inspection team have with them a copy of TRANSFIELD Emergency Procedures?

10-Customer Service

  • Did the Inspectors introduce themselves correctly?

  • Did they explain what their work procedure would be?

  • Did they show their ID badges.

  • Did they inform the householder that the power will be turned off for a short period of time?

  • Did they inform the householder that they require access to all rooms in the house?

  • Did they ask permission to take photographs?

11-Asbestos Work.

  • Is there an asbestos drop sheet and Plastic Isolation Tube Set Up in Work Area?

  • Is the Inspector wearing the correct asbestos PPE.

  • Are the correct asbestos procedures being followed?

  • Are they using the correct asbestos paperwork?

  • Was an Asbestos Approval Number obtained prior to starting work?

  • Did they double bag the asbestos PPE and any other contaminated material?

Roof Work.

  • Will the Inspectors be accessing the Ceiling thru the Roof?

  • Have the Inspectors chosen a Safe Spot to Set up Their Ladder?

  • Have They Set Up a Secure Anchor Point/s?

  • Is The Ladder Tied Off and Secure?

  • Have they Filled out the Roofing Paperwork?

  • Are They Wearing a Correctly Fitted Safety Harness?

Safety Harnesses

  • Is the Team Carrying a Safety Harness?

  • Does the Team Have a Tin Roof Anchor?

  • Does the Team have an Extra Tie Off Sling?

  • Does the Team have an Extra Intermediate Tin Roof Anchor?

  • Does the Team have an Extra Karibener?
