Employee details

  • Is anyone showing symptoms of COVID-19 (high temperature, dry cough, loss of smell, shortness of breath sore throat, sneezing, running nose)?

  • STOP - This person (or persons) must immediately go home. They need to communicate with their GP or book themselves in for a COVID-19 testing. They need to remain at home in their isolation bubble until the results of the COVID-19 test have been released or until their doctor says they are fit and able to return to work.

  • Has anyone been exposed to a person with a suspected or confirmed case of COVID-19?

  • STOP - This person (or persons) must immediately go home. They need to communicate with their GP or book themselves in for a COVID-19 testing. They need to remain at home in their isolation bubble until the results of the COVID-19 test have been released or until their doctor says they are fit and able to return to work. If someone in their bubble is a suspected or confirmed case of COVID-19 then they cannot come to work until all in the bubble have been medically cleared.

  • What is today's work plan? (Discuss each workers task for the day and any issues that may be encountered. Document if someone is under training or supervision). REMEMBER 5 - 10 extra minutes spent discussing the plan for the day could save someones life!

  • Do all Trucks have Fully Charged/ Working RT

  • (On the last day of work) Did the work plan run safely and to plan?

  • (On the last day of work) Did every worker work safely and to the conditions?

  • (On the last day of work) Were there any health and safety issued raised?

  • Were there any Incidents or Near Misses yesterday (discuss and record, no matter how minor)

  • Have these been entered into the Incident, accident, Near Miss form

Machine maintenance Issues

  • Are there any Trucks that need maintenance

  • Do not operate and notify management Immediately

Attendance details (Crew manager/foreman on the day) WHILE IN COVID-19 LEVEL 3 THE MANAGER OR FOREMAN IS TO SIGN ON EVERYONE'S BEHALF. BY SIGNING THIS THE MANAGER OR FOREMAN DECLARES THE ABOVE IS TRUE AND THAT ALL WORKERS ON SITE HAVE BEEN A PART OF THE TAILGATE.
