All PPE must be checked every 7 weeks and the form should be saved and exported to the direct lIne manager as a PDF.
Team Leaders must ensure that this procedure is carried out by all staff within their team and that there is evidence of individual PPE checks every 7 weeks.
Every 14 weeks the Team Leader should perform a random check against the submitted form for 2 of their team for quality assurance.
If any item of equipment is missing or damaged a Team Leader must be informed and the Stores team must be emailed detailing deficiencies and actions taken.
I accept that is my responsibility for completing this form and to correctly inspect my PPE to ensure it is in full working order. Any deficiencies will be notified to my direct line manager.
