1.0 Training Program (Safety / Training Department)

  • Has the site communicated the proper use of PPE to all affected employees? 1910.132(d)(1)(ii)

  • Does the training identify that defective equipment may not be used? 1910.132(e)

  • Does the site have a training program that addresses the following items: <br>When PPE is necessary;<br>What PPE is necessary;<br>How to properly don, doff, adjust, and wear PPE;<br>The limitations of the PPE; and,<br>The proper care, maintenance, useful life and disposal of the PPE.

  • Are employees required to demonstrate knowledge of proper care and donning and doffing before allowing employees to perform work wearing the PPE? 1910.132(f)(2)

2.0 PPE Program / Maintenance Program (General / Maintenance)

  • Has the site completed an assessment of hazards to determine whether the PPE would be necessitated? 1910.132(d)(1)

  • Does the site have a fitting process for PPE to ensure employees have access to different types of PPE that would provide the best fit? 1910.132(d)(1)(iii)

  • Is specialty PPE provided to employees at no extra cost? 1910.132(h)(1)

3.0 PPE Knowledge (Visual Inspection)

  • Do employees know where to obtain the proper PPE for their jobs?

  • Do supervisors know where to obtain the proper PPE for their jobs?

  • Does the site ensure employees have adequate PPE for the job or taks they are performing?

  • Can employees properly demonstrate an understanding of how to properly use PPE? 1910.132(f)(2)

  • Do employees know how to properly inspect PPE and what to inspect?

  • Are employees fully aware of the proper PPE requirements for the task they are performing?

  • Are supervisors fully aware of the proper PPE requirements for the task they are performing?

4.0 Employee PPE Usage (Observation and Visual Inspection)

  • Is the proper use of PPE enforced by management? 1910.133(a)(1)

  • Are employees using the required gloves for their task?

  • Are employees using the required Glasses for their task?

  • Are gloves in good condition?

  • Are hard hats current being worn correctly and not out of date according to manufacturer guidelines? (This should be on the hard hat)

5.0 Documentation and Retention (Engineering / Maintenance / Safety)

  • Are hazard assessments documented via written certification that identifies the task/workplace evaluated, the person performing the evaluation and the date it was performed? 1910.132(d)(2)

  • Are new hire training documents retained for the period of employment? (per the century north america document retention program)

  • Are annual and retraining documents retained for period of employment? (per the century north america document retention program)
