Convoy Procedures

  • Was a Pre-Convoy meeting held?

Employees & Workers

  • Are employees in high noise areas wearing appropriate hearing protection i.e. Ear plugs, muffs or headset?

  • Are drivers' DOT Hours of logs current and legible?

Manifold

  • Is the blender tender and hydration operator actively monitoring their tub fluid levels?

  • When running proppant (sand) is the hopper being kept full?

  • Are backside operators wearing proper PPE?

  • Is the blender tender communicating with the treater, hydration operator & sandking operator?

  • Is the densitometer being open at the start of the job and closed & locked after the job?

  • Is the backup blender 100% rigged in?

Chem Add / Chem Float

  • Are operators wearing proper PPE i.e. apron, face shield, chemical resistant gloves?

  • Are updated SDS in cab and available upon request?

  • Are the required HazMat response books in cab and available upon request?

  • Does the operator understand and perform proper strapping procedures?

  • Is the trailer marked with the proper placarding?

  • Are all straps in good operational condition free of cuts abrasions etc?

Acid / Gel Transport

  • Does the operator have signed BOL?

  • Are operators wearing proper PPE?

  • Are hoses properly ran and free of leaks?

  • Is a updated SDS located in cab and available upon request?

  • Are currents HazMat response books in cab and available upon request?

  • Does the operator understand how to take a strap?

  • Does the operator understand proper spill procedures?

  • Does the trailer have the proper placard(s)?

  • Are all compartments properly closed and secured?

  • Does the operator understand how to properly flush lines?

  • Does the operator have signed BOL?

SandKing

  • Are operators wearing proper PPE?

  • Are safety chains properly attached to stingers?

  • Are guard rails up and in proper working order?

  • Are ladders in safe operational condition free of broken / cracked welds, missing bolts etc?

  • Is sand being cleaned off of walking surfaces?

  • Are operators actively monitoring sand feed and blender hopper level?

  • Have designated high silica areas been properly flagged / marked using blue caution tape?

  • Are precautions being taken to limit proppant on the ground i.e. diverter connections on stingers etc ?

  • Do the operators understand how to gauge sand compartments?

  • Is the area free of excessive amounts of proppant (Sand) on the ground?

Line Truck / Boom Truck

  • Are all required inspections current?

  • Is all iron organized and neatly placed on line truck?

  • Are braces and racks accounted for and in good working order?

  • Are controls labeled and proper working order?

  • Is the cable in good operational condition free of frays and kinks?

  • Is the safety latch present on lifting hook and in operational condition?

  • Are the stabilization legs properly deployed prior to lifting operations?

  • Are lifting straps / cables in good operating condition?

  • Are tag lines being used and in good operating condition?

  • Certified crane operators operating the crane?

  • Is the line truck positioned at a safe proper distance from the well?

  • Are the proper amount of lifting straps being used to secure the load during lifting operations?

Pumps

  • Is the iron banded and in date?

  • Iron & hoses in good operating condition condition and free of leaks?

  • Are pumps primed to the blender tub?

  • Are pumps actively tended i.e. greasers kept full etc.?

  • Is there sufficient spacing between trucks to allow servicing and unobstructed escape path in event of emergency?

  • Are grease traps empty and not overflowing?

  • Have hoses and iron been properly secured after rig down?

TCV / DCT

  • Have the leveling legs been properly deployed?

  • Have the stairs and hand rails been properly deployed?

  • Are stairs and hand rails in proper operating condition free of defects or damage?

  • Are all lights in proper working order?

  • Are only authorized employees in the TCV?

  • Is the unit clean and presentable i.e. no trash or unnecessary items laying about?

  • Is the unit properly stocked with PPE, Caution Tape and a stocked first aid kit?

Incident & Location Information

  • Was job incident free of any on-site spills (Environmental Releases)?

  • Was job incident free from any injuries or reported near misses?
