  • Are there any personal factors that may cause you to be fatigued and have you discussed these with your supervisor?

Alcohol and Drugs

  • Are you taking any medication (prescription or over the counter medication) that may affect your ability to perform your duties as a rail safety worker?

  • Have you reported this to your supervisor?

  • Do you understand the John Holland Drug and Alcohol Policy?

  • What would you do if you had a big night out the night before your shift?

  • When was the last time you were Drug tested?

  • When was the last time you were Alcohol tested?
