General Safety

  • Is the NO food or drink rule being enforced and followed in laboratory?<br><br>Deficiency Response: Food and/or drinks should not be stored or consumed inside a laboratory. This practice reduces the potential of of someone ingesting a chemical, biological, or radiological agent.

  • Is laboratory free from housekeeping problems which can cause significant safety concerns? <br><br>Deficiency Response: Laboratories should be kept clean and orderly to reduce the risk from potentially hazardous situations.

  • Is Personal Protective Equipment (PPE) being used? <br><br>Deficiency Response: Recommend using protective clothing for protection against exposure to chemical, biological, or radiological agents.

  • Have safety concerns expressed previously been addressed?<br><br>Deficiency Response: Environmental Safety will be addressing the safety concerns expressed by the occupants during our visit to your laboratory. (List concern)

  • Have previous deficiencies been resolved? Note any repeat deficiencies as such.

Physical Safety

  • Fire egress with no obstructions observed in laboratory? A 3-foot wide pathway must be maintained to ensure sufficient egress.

  • Are fire extinguisher available and inspection current?

  • Are heat sources separated from combustibles? Recommend separating heat source(s) from combustible materials to reduce fire hazard.

  • Is laboratory free from extension cords used as permanent wiring? Recommend new outlets to reduce circuit overloading.

  • Is laboratory free of any frayed or damaged electrical cords? Damaged electrical cords should be replaced.

  • Is laboratory free of any evidence of trip hazards? Recommend removing walkway trip hazards for safe passage.

  • Is electrical panel free from obstruction? Electrical panels must have 36 inches of unobstructed area.

Chemical Safety

  • Is a Material Safety Data Sheet (MSDS) binder present in laboratory?

  • Are chemicals separated by combustible characteristics? (e.g. acids, bases, flammables, etc.)

  • Are chemical containers properly labeled?

  • Are corrosive chemicals properly stored? (e.g. sulfuric acid, nitric acid, sodium hydroxide)

  • Are flammable chemicals properly stored in flammable cabinets?

  • Are hazardous chemicals stored below eye level?

  • Are safety shower(s) and eyewash(s) stations properly working or unobstructed?

  • Is chemical fume hood sash closed when not in use?

Chemical Waste

  • Are chemical waste containers capped when not in use?

  • Are hazardous waste containers properly labeled as "Hazardous Waste"?

  • Is hazardous waste being properly stored?

Biohazard Waste

  • Are biological waste containers labeled properly? Box must be labeled with biohazard symbol.

  • Are sharps disposed in appropriate sharps containers?
