WHS Observation Checklist

Section 1: Scope of Work & JSA Compliance

  • Familiarise yourself with the tasks outlined in the JSA before visiting the site.
    Observe if workers are following procedures and are sticking to the tasks specified in the JSA.

  • Are workers performing tasks within the scope of the JSA?

  • If you observe tasks that present a high risk to worker health and safety—such as a worker operating a forklift without a license or using machinery they are not trained for—instruct the worker to immediately cease the task. Then consult with the client to address the matter and document all actions taken for rectification in the notes section of the WHS Observation Checklist. Section 2: Worker Consultation

  • Engage with workers in a respectful and confidential manner. Ask open-ended questions like, "Do you feel safe performing your tasks?" or "Are there any concerns about your work environment you would like to share?"

  • Do workers have any concerns about their health and safety?

  • Document the specific concerns in the notes section and develop actions to address them.

Section 3: Personal Protective Equipment (PPE)

  • Ensure you know what type of PPE is specified in the JSA for different tasks.

  • Are all workers wearing the appropriate PPE as per the JSA?

  • Document the specific concerns in the notes section and develop actions to address them.

Section 4: New Risks & Hazards

  • Be vigilant about any new equipment, substances, or changes in the work environment that weren't present during the last assessment. Pay close attention to traffic management, poor housekeeping, manual handling procedures, sun safety measures, rest breaks, workplace behaviours, and the condition of equipment and tools to identify potential health and safety risks.

  • Are there any new risks or hazards observed?

  • Document the specific hazards and associated risks in the notes section and develop actions to address them.

Escalation for WHS concerns

  • If any health and safety concerns have been identified, these must be promptly raised with the Regional/State Manager. Additionally, send an email copy of the WHS Observation Checklist and any action plans to both the HR Team and the WHS Consultant for further review and guidance.

  • Have any health and safety concerns been identified as part of this WHS Observation?

  • Inform the Regional/State Manager and email a copy of this report to the HR Team and the WHS Consultant for review and further action.
