Documentation

Start Up Documentation

  • Have the Point of Work Risk Assessments been done for the day? (Add in brief description of the works being done on the day of the inspection)

  • Have all representatives that form part of the inspection been inducted incl. the lead auditor?

Plant Documentation

  • Which plant are in use for the day? (Brief overview of which plant and machinery are in use)

  • Spot check some of the plant (Check driver/operator competencies, lifting certificates, daily/weekly/monthly inspections etc.)

Emergency Documentation

  • Have all staff been briefed on the emergency procedure and arrangements for the site?

  • Is there a copy of the ERP available and are emergency contact numbers displayed in the site office?

Permits/GATW

  • Are permits to work/GATW's available for the work being carried out on the day of the inspection?

  • Have permits/GATW's been signed by the PC and do they have valid dates?

RAMS

  • Spot check a task for the day with the risk assessment and method statement submitted

  • Is the task being carried out safely and as per the risk assessment?

Site Inspection

Welfare

  • Suitable welfare in place for personnel and visitors? (Check running water, flushable toilets, drying rooms etc.)

  • Are welfare facilities in good condition and clean/tidy?

Store rooms/Containers

  • Are store rooms and containers clearly marked with which type of store it is? e.g COSHH, General, Liftin Equipment etc.

  • Are they clean/tidy with good housekeeping practiced? Everything has a place and everything is in it's place?

  • Are store rooms easily accessible with steps or are there any tripping hazards present accessing the container?

  • Are doors wedged open safely?

Excavations

  • Are excavations barricaded safely and sufficiently?

  • Are signage displayed where excavations are present?

  • Are any excavations filled with water? If so, is it from recent rain/drainage (ask the contractor why the excavation is filled)

  • Are deep excavations shored or sloped where necessary?

  • Are plant/machinery working too close to the edge of excavations? (While persons are working inside the excavations)

  • Are too many personnel working inside the excavations?

  • Do personnel have safe means of access into and out of the excavation?

Security/Access/Egress

  • Is the site suitable fenced off with applicable warning signs posted?

  • Are all persons signed in and out at the security entrance?

  • Pedestrian walkways clearly marked and identified? Are they safe and well established?

  • Designated parking area available and are persons reverse parking?

Working at heights

  • Are there any working at heights currently taking place

  • Are all persons working at height wearing the appropriate fall arrest equipment?

  • Have the fall arrest equipment been inspected?

  • Spot check a staff member regarding knowledge of working at heights and what procedures are in place.

Personal Protective Equipment

  • Are all staff and visitors wearing the appropriate PPE

  • Are personnel wearing the right PPE for the tasks they are performing

Plant & Machinery

  • Are plant in good and safe working condition?

  • Do plant and machinery not in use have plant nappies/drip trays stored underneath?
