Equipment Use

  • Are the correct tools being used for the job?

  • Is the person competent with the equipment being used? and is it being used correctly?

Adherence to procedures/guidelines

  • Is the daily review meeting effective?

  • Safe job planning?

  • Has the appropriate level of SJP been used for this job?

  • Is the SJP completed and site specific?

  • Is the SJP signed on by contractors and all water corp staff?

  • Risk assessment and controls for hazards are in place?

Traffic Management

  • A manager or someone with basic traffic management training is on site?

Lone Worker

  • Understanding of lone worker definition and use of LWMS including report back intervals?

  • Assess supervisor use of monitoring LWMS usage?

PPEC usage

  • PPEC in good condition and being worn?

  • Long sleeve protective clothing worn?

  • High visibility clothing worn?

  • Safety glasses and gloves worn?

  • Did the person adjust their PPE to comply with safety standards?

  • Are adequate resources in place for purchasing of new items ?

Body position and manual handling?

  • Is the person placing any part of the body in a position that may result in an injury unnecessarily ?

  • Are operators aware of line of fire? (eg struck by moving objects, plant and equipment or traffic)

  • Does the work include regular use of awkward/restricted postures?(stooping , bending and twisting, working above head height, working away from the body)

  • If high manual handling factors are operators trying to mitigate these factors using good techniques, positioning etc of body, mechanical assist, assistance ?

Vehicle and Plant

  • Is vehicle and Plant housekeeping to a good standard?

  • Seat covers in use and fitted by dealership?

  • Ticketed and competent operators?

  • Condition of trailers and checks in place?

  • Are vehicle inspections conducted?

  • Is there a process for additional vehicle safety options?

  • Is the equipment purchased and fitted through fleet coordinator ?

  • Is driver safety and fatigue managed ?
