Physical Controls

  • Are physical barriers (e.g., fences, guardrails, bollards) in place to separate pedestrians from PME operating zones? (Mark as 'no' if there are areas where there clearly should be barriers but there are none or a lower-level control is in place)

  • Are the barriers and walkways color-coded according to standards (e.g., orange barriers, green walkways with yellow borders)?

  • Are pedestrian crossings provided where required, and are they clearly marked with black and white paint?

  • Where physical barriers are not feasible, are walkways marked appropriately with yellow borders, pedestrian icons, and directional signs?

  • Are "Forklifts in Use," "No Entry," "Truck Driver Waiting Area," and speed limit signs prominently displayed around PME zones? (Forklift signs should be in place at entryways to PME operating areas)

  • Are there Exclusion Zone explanation posters displayed near the exclusion zone and in the truck driver waiting area?

  • Are exclusion zones marked around loading/unloading areas, with red boxes extending at least 3 meters from the truck?

  • Are temporary barriers (e.g., cones, barrier tapes) used effectively when permanent exclusion zones cannot be established?

  • Is a designated truck driver waiting area clearly marked, adequately barricaded, and positioned to allow visibility of the loading/unloading process?

  • Are mirrors, windows around entrances, and warning lights in place to enhance visibility, especially around corners and blind spots? (check around warehouse door entrances, and blind spots into racking systems or product storage areas or pallet yards etc)

  • Are speed humps installed in pedestrian-prone areas to control PME and vehicle speed? (Check car park areas, pedestrian entrances to the site, prior to pedestrian crossings etc)

  • Is high-visibility clothing with reflective tape required and enforced for personnel working in operational zones?

  • Are PME vehicles equipped with visibility enhancements such as flashing beacon strobe lights and zone area beam light systems?

Operational Procedures

  • Is there a site-specific traffic management plan, and does it include pedestrian walkways, exclusion zones, loading areas, and car parks?

  • Are SOPs in place for specific PME operations, such as forklift usage, refuelling, truck loading/unloading, and WAVE machine operation?

  • Are there procedures in place to ensure that only PME or a pedestrian can occupy an exclusion zone at any given time and are those rules being applied? (will require some observations to ascertain if workers and truck drivers are complying with the rules - also ask some workers and PME operators to explain what the exclusion zone rules are)

  • Is there a 3-metre separation rule between PME and pedestrians in open areas, with clear guidelines on when exceptions apply, and are the rules being complied with? (Will require some observations to ascertain if the rules is being complied with - if unable to observe this, then ask some workers and PME operators to explain what the rules are)

  • Are there established rules for pedestrians approaching PME, including the need for eye contact, PME to stop and apply brakes, and signals before approaching?

  • Are operators required to stop PME operations, lower tines, apply brakes, and report any incidents of accidental interactions within the exclusion zone? (Ask some workers and PME operators to explain the rules)

  • Is a ‘safe lifting zone’ around shipping containers implemented, with cones or spotters in place when necessary? (If practice not observed then ask some workers (or truck drivers) what the rules are)

  • Are pedestrians given the right of way in open areas such as car parks, loading bays, and crossing points? (If unable to observe this during the audit, then ask some workers and PME operators what the give way rules are)

Training & Competency

  • Have all forklift operators completed the external training to NZQA Unit Standard 18496? (Dip sample some operators)

  • Have all forklift operators completed SOP training for the forklift operation?

  • Have all forklift operators completed a site-specific induction relevant to their roles?

  • Have all truck drivers completed the site-specific contractor induction?

  • Do truck drivers have a copy of the SOP for loading/unloading?

Communication & Monitoring

  • Are informative posters reinforcing PME and pedestrian interaction standards displayed around the site? (check for 3-metre rule posters, approaching PME posters, Exclusion zone rules posters)

  • Are workers, Team Leaders and Managers reporting near-misses, interactions within exclusion zones, and other PME-related incidents into the H&S incident reporting system? (Mark as 'no' if there are no incidents recorded yet you have observed incidents, non-compliance to rules, or breaches, during the audit)
