  • Do you have 1 x QSHE154 Employee Information sheet

  • Do you have disposable gloves and paper towels?

  • Is the that Alcomate senor is within its calibration date?

  • Does the Alcomate has sufficient battery life?(see icon on display)

  • Do you have at least 6 mouth pieces for the Alcomate?

  • Have you prepared the toilet?

  • Remove all accessories from the toilet area and check for any hidden materials. Put bleach, coloured dye or washing up liquid in the bowl to reduce the risk of sample tampering

Explain the following procedures to the employee;

  • If the individual is not known to the assessor, obtain photo ID from employee (driving license/passport) - take a copy for file

  • Have you given the employee QSHE154 “EMPLOYEE INFORMATION SHEET”

  • Have you asked, confirmed and recorded any substances taken by the employee in the past two weeks?

  • Have you explained that failure to provide a sample or consent to a sample will be recorded?

  • Have you explained that if a red band fails to appear next to one category, the test is positive?

  • Have you explained that if the sample is positive the employee will suspended with pay and the sample will be sent to lab for analysis & confirmation?

  • You can now start the testing process by completing the consent form QSHE 155

  • What are the Alcometer test results? (Include image of results)

  • What are the results of the urine test cup? (Take photo of results)

  • If the result is positive - a chain of custody pack is required and Drug aware should be contacted to inform them that a sample is being sent to them.
    If the result is negative - the employee can return to work and paperwork is submitted to HR
