Job Site Documentation

  • Has a Daily Job Hazard Analysis been completed and onsite? <br>If answer is no job shall be shut down immediately and crew is to hold their daily meeting.

  • Rate the completeness of the JSA with zero being the worst and 10 the best. JSA should include but is not limited to all daily scopes of work being performed, the hazards, and preventive measures taken.

  • Is rescue plan onsite? This is for aerial work?

  • Has property owner been notified that crew will be onsite? Schools, daycare, businesses etc...

  • Has crew checked in with the NOC?

  • Is current OSHA site sign with contractors name and phone number hung up and in view? <br>If no sign is onsite site shall be shut down until one is provided. OSHA 1910.145

  • Is an emergency action plan onsite with local emergency numbers and maps?

  • Have RF measurements been taken and documented?

  • Is RF monitor calibration sheet on site and valid? These are generally two years.

Certifications

  • Certifications are to be either the originals or a paper copy only. No digital copies!

P.P.E

  • Are hard hat being worn while onsite? Hard hats have a maximum of 5years. Check the dates. If expired have them replace immediately.

  • Is appropriate clothing being worn while onsite? Pants, steel toe boots, shirts with sleeves, etc...

  • Are safety glasses in accordance with ANSI Z-87.1 being worn?

  • Are gloves being worn when applicable?

  • Is ear protection available and being worn? 85db and above.

  • Are Hi Visibility vests needed for traffic or when heavy equipment is being used?

Fall Protection

  • Is 100% fall protection being 6ft and above 100% of the time? Individual(s) not complying shall be removed immediately from site and will no longer be allowed to work on future sabre sites.

  • Do employees who are exposed to falls have proper training?

  • Is fall protection being inspected daily and documented when being used? Harness life is five years from first use. Inspections tags are located on harness.

  • Are carabiners self closing and locking? If at risk equipment shall be tagged and put out of service.

Rigging

  • Will there be rigging of equipment, materials, or tools? Yes or NA

  • Does crew have Dynometer onsite for load test? If applicable. If no than crew shall not pick material until one is acquired.

  • Has a load test been performed and documented prior to any material lifts daily? This is a 1.5 times the max pick for the day. If no crew shall stop work and preform said test.

  • Does the rigging sequence match the rigging plan?<br>

  • Does the competent rigger know how to calculate numbers in rigging plan?

  • If answer is no advise they need to learn this ASAP as future sites will be shut down. If they are able to preform this have them write how it's configured on the back of the rigging plan.

  • Do all blocks rated for the capacity of the load?

  • Are blocks the correct size for rope being used?

  • Do all blocks labeled for weight rope size etc...

  • Are carabiners being used anywhere in the rigging? If at risk job site shall be shut down and corrected before work resumes.

  • Is the capstan winch mount correct per rigging plan?

  • Does crew have rescue bag and first aid kit at or around base of tower? If no site shall be shut down until provided.

Hoists (Capstan)

  • Is operator qualified?

  • Are hoists secured properly and properly anchored?

  • Does capstan winch have rope bar attached? If no, winch shall not be used until provided. Winch may have rope lock installed as additional safety but not as a replacement for bar.

  • Is capstan winch in good working order?

Cranes

Ladders

  • Are ladders onsite in good working condition?

  • Has a daily pre-work inspection been completed of all ladders in use, and is the inspection noted in the daily?

  • Are extension ladders tied off or staked at ground to prevent slipping? If no correct onsite.

  • Are A-frame ladders set up correctly with legs fully opened and locked in place?

  • Are extension ladders set up at the proper slope of 4:1?

  • Do extension ladders extend at least 36" above the landing level?

  • Is extension ladder tied off of staked to prevent slipping? If no than correct onsite.

Hot work/ Welding

  • Any tool that emits sparks in or around compound is considered hot work?

  • Was a pre construction meeting held prior to any hot work being performed?

  • Is the cutting and welding plan on site and signed by all applicable persons?

  • Is the hot work permit on site and signed by all applicable persons? CWI, CCI CM, Etc...

  • Is all combustible items identified in C&W plan?

  • Have all gas/ fuel line in and around site been checked for leaks and documented prior to hot wor beginning? If at risk site work shall stop until corrected.

  • Is there a minimum 300 gal of water for suppression? Hose needs to be at hot work area.
