Inspection and Audit Checklist

Pre Inspection and Audit Instructions

  • Use this audit to assess the following aspects of this site as Safe, At Risk or Not Applicable. Unsafe response indicate that attention is required to bring items to a satisfactory standard. Notes and photos should be attached where appropriate, particularly to At Risk items.

  • Have you reviewed the last Safety Inspection and Audit document to ensure all actions identified have been completed and signed off?

  • Any outstanding actions add to this template for review, follow up and sign off

  • Every effort should be made to seek input from site personnel including contractors, volunteers, etc. in identifying and controlling hazards during the inspection.

Emergency Management, including. First Aid

  • In this section, you must check all the emergency equipment and first aid kits/rooms to ensure that they remain fit for purpose and adequately maintained.

  • Are extinguishers & fire hoses clearly marked as to their location?

  • Are all extinguishers & fire hoses service tags up to date (checked every 6 months)?

  • Are the Evacuation Maps up to date and the emergency exits clearly marked?

  • Are the Emergency Exits signs illuminated?

  • Are the emergency exits easily accessible, opened and clear of all obstructions?

  • When was the last drill conducted at this site

  • Are there Duress Alarms onsite?

  • Are they in good working order?

  • Are employees trained in the use of duress alarms?

  • Are the first aid rooms and kits onsite clearly marked and regularly maintained?

OHS Notice Boards

  • Is the OHS Notice Board updated

  • Are all the appropriate flyers, minutes, etc. visible? (i.e. Safety Bulletins/Alerts, Incident Reporting, If you are injured at work poster, sub-committee minutes, HSR flyer, First Aid, Wardens, etc.)

Personal Protective Equipment (PPE)

  • Do you wish to examine PPE on this safety inspection and audit?

  • Appropriate Clothing (in safe condition) (including high vis)

  • Foot Protection (fit for task, worn by crew and in safe condition)

  • Gloves (available, worn by crew and in safe condition)

  • Hard Hats (available, worn by crew and in safe condition)

  • Hearing Protection (available, worn by crew and in safe condition)

  • Safety Glasses with Side Shields (available, worn by crew and in safe condition)

  • Face Shield / Goggles (available, worn by crew and in safe condition)

  • Respiratory Protection (available, worn by crew and in safe condition)

Tools and Equipment

  • Do you wish to examine tools and equipment on this safety inspection and audit?

  • General condition of tools

  • Has a Risk Assessment and Safe Operating Procedure been created for the tools and equipment being used?

  • Tools and equipment inspections are taking place and being recorded appropriately

  • Right tools are being used for tasks

  • Employees are trained to be competent in the tools appropriate for their work tasks

Work Area - Electrical & Lighting

  • Do you wish to examine electrical and lighting on this safety inspection and audit?

  • Has the electrical equipment been tested and tagged?

  • Are plugs, sockets, switches, leads, etc. in good working order?

  • Are the leads all secure and neatly organised, not causing any trip hazards?

  • Is there adequate illumination (natural and artificial) for the tasks being completed?

Work Area - General

  • General Housekeeping (neat, tidy, dust free)

  • Walkways (free of debris and obstructions)

  • Is the work area equipment suitable for the tasks being completed?

  • Is the temperature of the workspace acceptable?

  • Flooring adequate (coverings, etc.)

  • Storage and racks stored appropriately, to minimise manual handling issues (where reasonably practicable)?

Work Area - Amenities

  • Are amenities clean and tidy, including toilets, washrooms, meal areas, lockers, etc.

  • Is there running hot and cold water?

  • Is the boiling water/cold water taps in working order?

Chemical Handling

  • Chemical handling includes general household items as well such as:
    - Fly Spray
    - Air Fresheners
    - Detergents
    - White Board cleaner
    - Carpet Cleaners

  • Do you wish to check chemical handling on this safety inspection and audit?

  • Are all substances stored in labelled containers?

  • Are all substance stored in lockable containers/cupboards away from children?

  • Are Safety Data Sheets (SDS) up to date and relevant?

  • Do employees know how to read an SDS and where they are located?

Manual Handling

  • Is manual handling required to be completed while in this workspace?

  • Are employees adequately trained on manual handling techniques?

Child Safety

  • Poster/Notice board pins out of children’s reach?

  • Furniture in good condition and away from walkways?

  • Fans/leads are out of children’s reach?

Voluneers

  • If Volunteers use this site, have they been through Council’s “Volunteer Management System (VMS)?

PROPERTY RISKS

Observed Storm Exposure

  • Are the gutters/down pipes appropriately maintained?

  • Are there stains on the internal walls from storm damage

  • Are there overhanging trees presenting a risk to the building

Observed Burglary, Theft or Vandalism

  • Is access possible for unauthorised personnel?

  • Are there poorly secured doors, windows or skylights?

  • Is portable equipment visible or accessible to the public?

Observed Arson

  • Are wheelie bins stored near buildings?

  • Is there excessive dry vegetation near the building?

  • Is there public access to combustibles?

  • Is there excessive flammable liquid / gases stored inside the building?

General Comments

  • Any further comments or recommendations arising from this inspection?

  • Add any additional relevant photos

  • Have all required corrective actions been added as Actions to this inspection?

  • Please add any corrective actions to the appropriate questions above before completing this report
