HazardCo pack on site, site specific safety plan on site and Hazard board erected.

Signage correctly in pace and up to date, fencing and barricades are in place, there is safe pedestrian access past site, clear area for loading and unloading deliveries and any construction areas separated from occupied areas

Induction given to client and sub-contractors self-inducted, had significant hazards explained and there is a completed emergency plan on site

PPE is being worn, is in good condition and being worn correctly (head/face, eyes/hearing skin, footwear, protective clothing)

Fall protection is in place, working at height guideline is being followed. Scaffold, platform edge protection etc and mobile platforms, safety nets etc are in place where required. Harnesses as last resort

Hazardous chemicals are being controlled and labelled, a hazardous substance register is available and Material safety data sheets are available

Health, a trained first aider, first aid kit, running water and toilet are available on site

Scaffolding is correctly braced and stabilised at 3.1 height to base ratio. Scaffold is on firm foundations, not subsiding, plumb and level, base plates are in place and there is ladder access. Platform is 675mm wide, planks are tied down. All edge protection is in place. Scaffold is tagged & under 300mm from working face

Noise level is below 85 decibels if not hearing protection is being worn

Tools and materials are kept back from edge and prevented from falling/secured

Ladders no domestic on site, secured against movement or footed, in good condition, extended 1m above platform – 4.1 angle, no loads being carried while climbing, 3 points of contact

Electricity are all leads and RCDs are tagged and tested, no temporary repairs or exposed wires, all are clear of walkways, high traffic

Hand and power tools are in good condition, tagged, tested and being maintained. Used correctly, guards and isolation in place, flying debris controlled

Excavations the soil stability known, no water accumulation, existing services known and isolated, material 600mm from edge. Trenching over 1.5m notified to MBIE. Shoring is in place.

Gas Cutting/Welding flash/burns controlled with PPE and shields, fumes controlled with ventilation, leads in good condition, out of water, blow back valves, secured upright cylinders, combustible materials controlled

Site is clear or debris/rubbish, there are no trip hazards, clear access to work areas and bins being used
