  • Induction and Orientation-Did the site induction and orientation outline site hazards and current operating conditions?

  • First Impressions-are important, are operatives wearing correct PPE, is parking well defined, is signage clear and appropriate, is housekeeping adequate, is site secured appropriately etc.?

  • High Risk Activities-Are any high risk activities being undertaken on site? Ie. Contractors undertaking works, Working at height, Confined space entry, Excavations, Mobile plant lifting operations, Isolation and tagging, Traffic management etc. Are appropriate permits/inductions in place I.e. (CSE, CTW)? Has appropriate "Safe job planning" been initiated and documented for any of the above tasks?

  • Hazard/incident identification & rectification- discuss with the operator/s any recent incidents that have occurred or any hazards that have been identified. Discussion based on the importance of reporting and "if we don't know about we cannot fix it". Have they had feedback on how hazard resolved or summary of incident.

  • General observations-Outline any issues or positive observations which you feel are worthy of note from the visit. E.g. Operative observed rectifying a hazard.
