Is the permit to work, SWMS, JHA, and take 5 available and understood?

Have all team members been trained in W@H?

Does the area have a high standard of housekeeping?

Has a W@H permit, dropped object inspection been completed for the area and actions addressed prior to starting work?

Have all areas where an object may fall through been covered to protect dropped objects? Are blankets and mono flex available?

Do all members have adequate personal protective equipment, including Falls arrest Equipment?

Has an exclusion zone been set up below the work area where required? Is there adequate signage warning of people working above?

Have all tools and equipment required for the task been fitted with tool lanyards
