The MPFM Assembly

Are both nylon blanking plates (top and bottom) of the MPFM bore fitted, bolted down and displaying the trefoil radiation symbol in accordance with the THI?

Is there a radiation trefoil symbol attached directly to the MPFM's source holder and in good condition?

Is there a printed information sheet attached to the MPFM which shows the unique identification number allocated?

Do all the serial numbers on the printed information sheet match the shipping documentation?

Are there two tally plates attached to the MPFM? One plate for the overall assembly, the other plate specifically for the source.
