Does this audit reflect all the Controls from the most recent Hazard and Risk Register ?

Is there adherence to procedures for Confined Space Entry and Hot Work ?

Is the tank fully vented and declared safe for entry?

is appropriate PPE worn, including bump cap or hard hat ?

Is there a permitting procedure for confined space and hot work ?

Is there Continuous monitoring of internal tank environment with gas monitor?

Does a stand-by person remain at entrance of tank and in contact with personnel inside tank at all times?

Is Radio contact with stand-by person and TNZ operators maintained?

Is work suspended if weather changes?

Low voltage lighting fitted with earth leakage device in use ?

Is there an emergency rescue plan developed ?

is emergency equipment immediately available at tank entrance? This will include SCBA equipment
