1. Project Planning

  • 1. Has an Appointed Person been appointed in writing and available for lifting activities?

  • 2. Is the Appointed Person based on site?

  • 3. Are lift plans and risk assessments suitable, available and approved by the Appointed Person for lifting activities on site.

  • 4. Is the lift plan briefing sheet signed by all members of the lifting team.

  • 5. Have temporary works and ground conditions been considered and risk assessed.

2. Competency of Lifting Team

  • 1. Are Crane Supervisors and Slinger / Signallers trained, experienced and appointed.

  • 2. Does the Crane Supervisor hold a Slingers CPCS qualification.

  • Are Sub-Contractor training cards copied and stored on site (CSCS, JCI or SPA UKPIA)

  • 4. Do the Slinger / Signallers hold the correct category’s for the type of slinging / signalling duties they are required to carry out.

  • 5. Is the Crane driver competent (check CPCS card) with a valid safety critical worker medical.

  • 6. How many members of the lifting team are there and is it enough to carry out the works safely.

  • 7. Are members of the lifting team multi job operatives or dedicated.If multi job, what is their primary role.

  • 8. Do all parties where applicable wear something that clearly identifies them for that role.

3. Lifting Accessories

  • 1. Is a LOLER inspection regime in place with up to date records, a good history ad documents to support this.

  • 2. Are lifting accessories in good order.

  • 3. Is the lifting tackle tagged, including expiry dates.

  • 4. Are lifting accessories being used correctly.

  • 5. Are lifting accessories being stored correctly.

  • 6. Is there a lifting tackle quarantine locker.

  • 7. Is access and egress to lifting equipment available and suitable.

4. Permits

  • 1. Is there a permit to lift system used on this site.

  • 2. Are permits to lift used and show history of past permits.

  • 3. Are Telehandlers / Excavators covered by the lifting permits.

  • 4. Are delivery lorry loaders covered by lifting permits.

5. Lifting Loads

  • 1. Is the lift area identified, segregated with exclusion zones and controls in place for clash avoidance.<br>Are the clearly marked, briefed and understood.

  • 2. Are communications agreed and understood between operations for control of the lift.

  • 3. Are suitable load set down areas identified and used.

  • 4. If loads are landed on temporary decks (scaffolds, Titan, suspended slabs, cantidecks) is the SWL displayed.

  • 5. Are all loads being secured properly. Small items lifted in cages, loose items banded.

  • 6. When lifting loose loads, bundles of tubes, rebar or timber is a double wrap choked hitch used.

  • 7. Do delivery lorry loaders have the relevant certificate, are competency checks carried out and are records kept.

  • 8. Are safe material loading / unloading arrangements in place which reduce the need to climb onto flatbed lorries or their loads.

6. Excavators

  • 1. Are excavators used for lifting operations.

  • 2. If yes to the above, do they comply with all of the above questions.
