Part A - Pre-Start - Concreting Company Checks to Confirm Suitability

  • Regular plant maintenance, inspection and testing records<br><br>(Including daily checks, mfr. recommended inspections, major (6 yearly) inspections, thickness tests on pump lines, yearly crack tests)

  • Plant Risk Assessment

  • Generic or site specific SWMS

  • Worker training, HRW licences and CI cards

  • Any required registrations and transport permits

  • Signature of concreting company representative

Part B - Operator onsite pre-start check

  • Has the pumping equipment been inspected and checked prior to use to ensure it is in sound operating condition?

  • Is all the required safety equipment available?<br><br>(Including PPE, bollards/traffic cones, fire extinguisher, first aid kit)

  • Are all equipment manuals and safe operating instructions available on site?

  • Are all scheduled maintenance inspections and logbooks up to date and filled out correctly?

  • Is a SWMS available and has it been reviewed to include any site specific hazards?

  • Are all workers inducted into the site and the SWMS?

  • Is the pump set up location suitable in accordance with the SWMS and manufacturers instructions?

  • Are there adequate traffic management arrangements in place?

  • Is the equipment set up in accordance with the above?

  • Signature of the Pump Operator

Part C - Supervisor to check prior to site operation

  • Have all the above checks by the builder and contractor been completed and any deficiencies addressed?

  • Are there enough trained and competent workers available to safely use the equipment?

  • Are there adequate arrangements for supervision and to ensure safe systems of work are followed?

  • Name of the person responsible for supervising the operation.
