SECTION 2 – PAPERWORK & LANTRA / COMPLIANCE CHECKS ASSESSMENT

TM Layout - Does the TSCO undertsand the TM layout which is due to be installed? (and can the TSCO tell /evidence to you the following information?

  • What the timings for the closure are?

  • if there is a specific sequence that the closure needs to be installed and removed in?

  • Has the TSCO performed a competency & ratio check on the TM personnel?

  • Does the TSCO have access to the latest industry standards documentation?

Minimum standards

  • Is all the correct PPE worn by the TSCO, clean, tidy and fit for purpose with the correct company logo?

  • Does the TSCO have a copy of area specific minimum standards (where applicable) and octavius TM minimum standards?

  • Does the TSCO have the ability to communicate clearly in English? (both written and oral)

Site set up

  • Where a diversion route is installed, was this driven and checked by the TSCO prior to installation commencing?

  • Did the TSCO supervise the installation/removal? (only if safe place to view from)

  • Was a drive through carried out to ensure that the closure has been installed as per the traffic management design.

  • Was a drive past/drive through conducted prior to ensure safe and correctly installed before allowing access to the closure?

  • Were any hazards sought (and if found identified) during the installation/removal of the closure or once the closure had been installed? Please provide more details in the comments section?

  • Was a drive past/drive through conducted by the TSCO prior to ensure safe to hand back to the TM personnel?

SECTION 3 - BEHAVIOURAL & NON-TECHNICAL ASSESSMENT (Situational awareness)

  • What would the TSCO do if during the evening the temperature drops to -2 degrees (would this response be appropriate?)

  • When entering the closure where would the TSCO position His/Herself ? (is this safe and appropriate?)

Conscientiousness

  • Did the TSCO have a systematic and thorough approach

  • Did the TSCO carry out checks where applicable

  • Does the TSCO have a positive attitude towards rules compliance ? (Ask why they believe HIPO's still happen)

  • Does the TSCO Understand the purpose of their role and takes their responsibility seriously.

  • Have TM supplier RAMS been read and understood what requirements different TM suppliers may have in place, (I.e. Chevron SWP 026 there should be a breifing prior to lifting the closure, no other TM provider requires this.)

  • Did the TSCO communicate well in their brieifngs and conversations to encourage and check on listening (Are people actively listening and did the TSCO encourage and listen to feedback)

  • Was there opportunity for the group to ask questions in relation to the breifing

  • Does the TSCO understand what documentation is required from them in terms of post-shift reporting with photo's if necessary - and are they preparing to provide this report/have reported as necessary?
