SSA Skills Demonstration Checklist

 • Standard precautions using proper PPE

 • How to prepare solution measurements, time, and temp for soaking instruments and proper PPE. (First put on my PPE then run water till it is warm and is between 77 and 130 degrees test the water with my thermometer , once the water is up to temp measure 1 gallon of water and then 1 ounce of enzymatic cleaner, the instruments must soak for a minim of 2 minutes.)
• How to wash instruments. (Rinse the pre klens off of the instrument and place in my enzymatic solution for a minimum of 2 minutes. After soaking I would wash under the water [if I had to I would use a disposable brush ,disposing of the brush at the end of the day] to scrub off any stuck bio burden on the instrument then I would rinse the instrument off and place on the clean side to dry.)

 • Proper technique for rinsing red containers and using Oxyvir TB to sanitize containers (container contact time). (Rinse the red containers with warm water ensuring all of the Pre Klenz has been removed. Then I would place the red containers in the designated area and spray copiously with Oxyvir TB ensuring that it is on the container for 5 minutes.)

 • Proper way to package instruments. (Place instrument into steri peel handle first (ensuring that the box locks are not locked and the scissors are open. Place an indicator into the steri peel and seal shut.)
• Proper way to load and correct setting for load. (If only one steri peel I would lay it down but if more then 1 would set them on there edge paper to plastic and never layering like pancakes. select the pouches cycle on the sterilizer.)

 • How to clean sterilizer and maintenance log (D, W, M). (Daily mix a little speed clean with a little distilled water and wipe down the outside of the sterilizer and log on the maintenance log when completed. Weekly drain the sterilizer and refill the sterilizer with distilled water remove the racks and wipe down insid and
outside of the sterilizer, also wipe the gasket replace the racks into the sterilizer and log the weekly cleaning in the maintenance log.
Monthly drain and fill the sterilizer with distilled water remove the racks and place 1 ounce of speed clean in the bottom of the sterilizer and run 1 pouch load. when completed drain and refill the sterilizer with distilled water and run 1 unwrapped cycle wait till the sterilizer comes up to pressure (the times will vary) then open the pressure relief valve long enough to visualize steam removal then close valve and let the cycle complete. When it is finished and cooled. Once again drain and refill the sterilizer also remove the lint traps from the bottom and back wall removing any visual debri and placing them back into their spots.. then fill out the maintenance logs.)
• How often are biologicals are run per week? (Ex. Biologicals are run twice a week at the clinics Mondays and Wednesday).

 • Package biological and placement in sterilizer. (Place a biological into the steri peel pouch with an indicator and seal the pouch . May run the biological with or without instruments. Place biological in the lower center tray.)

 • Verification of biological after load for change

 • Verification of correct control biological and sterilized biological.(After the biological has been ran I would select a control and compare the lot # and expiration Date to ensure they are the same . Then I would look to see if the BI changed from pink to brown. I would write a C on the top of the BI that I am using for my control and the date. Crack and place the control into the incubator then I would write the date, load # sterilizer # in the BI I ran in the sterilizer. Crack and place the BI into the incubator. Fill out the sterilization envelopes.)
• Proper logging of biological on sterilization envelopes. (After the incubation period has been completed , then go back and log either a negative or a positive test result on the sterilizer envelopes.)
• Knowledge of when to read and log biological in sterilization envelopes

 • Staff can show you the manufacturers guidelines

 • Staff can show you the Safety Data Sheets for chemicals used

 • Staff can locate an eyewash station and it can be reached within 10 seconds/55 feet, unobstructed path.

 • Review documentation of eyewash checks on the tag at the station for the last month. Does documentation include weekly checks?

 • Review staff education files of at least 1 individual who performs sterilization. Does documentation include education and competence assessment of the sterilization process being performed, including specifics for the item being disinfected and the disinfectant being used? Was competence assessment documented during orientation and at defined frequencies?
