ELECTRICAL

  • Is all temporary power GFCI regularly tested, maintained and logged?

  • Is temporary lighting adequate?

Working at Heights

  • Are Ladders inspected regularly and logged? Other than each use each day.

  • Are ladders red tagged and returned to office ASAP when defects or damages are discovered? DO NOT ATTEMPT ANY REPAIRS AT ALL.

  • Pratice ladder safety 3 points of contact and do not stand on last rung or top?

  • No metallic ladders in use.

SCAFFOLDS

TRENCHING and EXCAVATIONS

  • Are CAZ's tagged with contact information? Use LOTO tags.

  • Is trench box being used?

  • Is there required ingress and egress?

  • Is depth in excess of 20 feet deep?

PPE

  • Hardhats worn?

  • Gloves to match task at hand?

  • does task require KEVLAR sleeves?

  • Hearing protection readily avaliable?

FIRE SUPPRESION
