  • Safety meetings?<br>Are safety meetings scheduled? Well attended? Documentation of attendance? Topics appropriate for the phase of construction?

  • Pre-task planning ?<br>Are pre-task forms being filled out? Completely, legibly, descriptive, signed by all participants?

  • Emergency & medical response?<br>Is there a crisis management plan with contacts listed? Are first aid boxes properly accessible and stocked? Is an AED available, accessible, battery charged, contacts not expired? Is there a map to a clinic and an emergency room posted?Grab & Go Trauma Kit available, stocked?

  • Fire prevention and extinguishers?<br>Are fire extinguishers posted throughout the project? Are they the right class of extinguisher? Have they been inspected? Are they charged? Accessible? In good working order?

  • Guarding falls: edges, floor, roof?<br>Can guard rails and covers hold at least 200#? Are hole coverings labeled? Are holes larger than 2" covered?

  • Forklift, scissor lift, man lifts?<br>Has the equipment been inspected? Has the user been trained and certified? Is the lift appropriate for the use? Is the lift on level ground? Are all the necessary safety guards in place?

  • Excavation and trenching?<br>Is the excavation or trench greater than 5' deep? Is a permit required? Is the soil type known for benching, sloping, or shoring? Is a ladder provided within 25' of all points in excavations greater than 4' deep?

  • Crane and rigging? (Not just tower cranes)<br>Is a crane plan in place? Has a communication system been established? Does the crane have sufficient capacity? Is weather a factor? Do we have copies of the crane inspection and certification? Do we have a copy of the operator's certification? Is the swing radius barricaded? Are ropes/slings/chokes/rigging in good condition?

  • Housekeeping?<br>Are access and egress ways free of debris? Are cords and other trips hazards abated? Is dunnage being handled more than necessary? Is debris being removed quickly enough?

  • Lighting / noise / dust / air quality?<br>Is there adequate light provided for the tasks being performed? For access or egress? Are the lights in good working condition? Is additional task lighting available? Does the noise level exceed exposure limits? Is hearing protection available? Is a hearing conservation program required?

  • Sanitation / water?<br>Are there enough toilets for the project? For each gender? Are washing facilities available? Are first aid and drinking water station separated to minimize blood-born pathogen contamination?

  • Access / signs / staging?<br>Are access routes being maintained through the project? For pedestrian, delivery, emergency routes? Are signs placed to notify pedestrians about changed routes, PPE requirements, etc? Are staging areas well maintained?
