  • Gas cylinders stored upright and secured with rope or chain?

Scaffold Safety

  • Are daily Scaffold inspections performed by a competend person?

  • Planks overlapped no less than 6" or more than 12" over end supports with toe board in place?

  • Tubing pinned properly and all cross bracing in place?

  • If scaffold height is 4X smallest base dimension, is system secured to structure?

  • Are guardrails in place? Full working Plattforms at each working level? Safe access provided to each working level?

  • Scaffold and components not overloaded? Is scaffold sytem plump and level?

  • Suspended scaffold systems using independent personal fall arrest system?

Fall Protection

  • Is a full body harness used where required?

  • 100% tie-off to a structural element supporting 5000lbs./person

Ladder Safety

  • Are air monitors being used, if excavation site is contaminated or suspected of contamination?

  • Are all permits up to date and is a competend person on site?

Electrical

  • Are temporary power panels and receptacles protected from the weather?

  • GFCI's being used for tools?

  • Are temporary lights rigged and secured to supports properly and with covers?

  • If overhead power lines are in areas, are operations maintaining required distance and insulation?

  • Is lock out/tag out program in effect?

  • Has a scetch of proposed temporary power distribution been submitted/accepted before installing?

Cranes

  • Has periodic inspection been performed prior to use on site (OSHA/EM385)?

  • Are equipment inspections done daily by a competend person?

  • Is crane operator qualified per OSHA or EM-385?

  • Are workes protected from the crane swing radius and prevented from passing under the load?

  • Are rigging cables and slings in good repair and free of kinks and cracks?

  • Is the crane on level ground and firm ground and outriggers is use with appropriated cribbing?

  • Is crane side loading prohibited?

  • Near electric power sources are rules followed for clearance/isolation in opeating zone?

  • Is crane equipped with anti two-block device if required?

Confined Space

  • Has entry plan submitted and accepted?

  • Is the atmosphere being monitored? Is space being ventilated?

  • Is space being ventilated?

  • Are entrants, attendants and entry supervisior properly trained

  • Is rescue/retrieval system in place?

  • Are daily entry permits postedat point of entryand signed by entry supervisior?

  • Is point of entry sign posted, "Danger Confined Space" ?

Roofing

  • Are Kettles at least 25 feet away from buildings?

  • Is a fall protection program in place and properly used?

  • Do modifications meet safety ratings per manufacturer?

  • Are safety lashings provided for high pressure hose connections?

  • Are workers clear of blind spots associated with mobile construction equipment?

  • Do aerial lifts have basket/platform with guardrail?

  • Are workers not extending over guardrails of aerial lifts. Climbing on guardrails?

  • Are articulating boom platforms (JLG types) used with Full Body Harness attached to boom or basket?

Demolition

  • Has demolition plan been submitted and accepted?

  • If waste is being dropped >6' is it in an enclosed chute and is area secured from traffic?

  • For building demolition, has notification being made to State having jurisdiction?

  • Are nails removed from scrap lumber/materials?

PPE

  • Workers wearing leather shoes with safety toe, long pants sleeved shirts ?

  • Are hard hats being worn?

  • Are safety glasses worn where appropriated?

  • Hearing protection where appropriated? (if you need to yell to have a conversation)

  • Respirators where appropriated, and workers being trained on using resprirators?

  • Impalment protection in place where workers could work above vertical impalement?

  • Is lighting adequate?

Abatement

  • Has abatement plan submitted and accepted?

  • Is independent air monitoring performed as required inside and outside barriers?

  • Is containment in place without integrity compromised?

  • Are employees using appropriated PPE?

  • If negative air is used, are fans used continously and monitored for pressure differential?

  • Has baseline been performed and necessary final clearance readings taken?

  • Are inspections by independent PQP performed prior to barrier removal?
