5.0 Electrical

  • Lockout/Tagout procedures used when needed?

  • Ground fault circuit interrupters (GFCI) being used?

6.0 Hand, Power, & Powder Actuated Tools

  • Hand tools inspected regularly?

  • Guards in place on grinders and tools?<br>

  • Right tool being used for the job at hand?

  • Operators of power actuated tools are licensed?

7.0 Fall Protection

  • Ground hole(s) & exposed edges are protected?

  • Guardrails and cables are in place and secured properly?

  • Employees have safety harnesses and used correctly?

  • Employees exposed to 6ft fall hazards are tied off?

  • Employees below protected from falling objects?

8.0 Ladders

  • Are all ladders Industrial strength? (Non Household rated, check label.)

  • Ladders are extended 36" above the landing?<br>

  • Are ladders in good condition?

  • If used for electrical work are they non conductive? (Wood or fibre glass.)

  • Used according to manufactures instructions?

  • For extension ladders are ropes, pulleys and treads in a good state of repair?

9.0 Floor & Wall Openings

  • All floors or deck holes are covered or barricaded and marked?

  • Perimeter protection is in place and maintained?

  • Hole and Deck covers are secured?

  • Materials stored away from edges?<br>

10.0 Trenches, Excavation, & Shoring

  • Competent person inspected trench daily and on hand?

  • Excavations are shored, benched, or sloped back?

  • Materials/spoil piles are shored at least two feet from the edge of the excavation?

  • Ladders provided every 25 ft in the trench?

  • Excavations over 20 ft in depth have engineer approval?

  • Equipment is a safe distance from the edge of the trench or excavation?

  • Walkways over trench are adequate and secure?

  • Shoring and sheeting as needed for soil classification and depth?

  • Fall protection into the trench has been addressed?

11.0 Material Handling

  • Materials are properly stored or stacked?

  • Employees are using proper lifting methods?

  • Tag lines are used to guild loads?

  • Proper number of workers for each person?

12.0 Welding & Burning

  • Gas cylinders stored upright?

  • Proper 20 ft separating distance between fuels and oxygen?

  • Welders wearing protective leather gear to prevent burns?

  • Shields utilized to protect other's from flash burns when necessary?

  • Fire watch being used were required?

  • Burning / welding goggles or shields being used?

  • Fire extinguisher are near by?

  • Hoses are in good condition?

  • Welding gas cylinders capped when not in use?

  • Carts provided when moving cylinders?

13.0 Cranes

  • Outriggers are extended and swing radius barricade in place?

  • Crane setup level and solid pads under outriggers?

  • Operator is familiar with load chart?

  • Hand signal charts are on site?

  • Safety latch provided and working properly on the hook?

  • Crane operator's daily inspection logs are up to date?
