A Yellow Card will be served upon an individual sub-contractor/employee where it is the opinion of Site Management that there is a failure to comply with Shropshire Homes Ltd Health & Safety Policy Manual/Safe system of work or legislation without reasonable grounds.
The Yellow Card will state the issuer, recipient, deficiencies found and recommendations for compliance, and copies will be held by the Health & Safety Manager, the person to whom it is issued & a copy will be sent to their employer for their records, where applicable.
The Yellow card will remain in place for a period of 12 months from the date of issue.
Below is a list of common contraventions:
• Failing to follow company procedures for Covid-19 Dependant on risk of harm
• Failure to wear compulsory PPE (Hard Hat, Hi-Viz) Yellow card for 1st failure.
• Failure to wear PPE as per Risk Assessment (Dust mask, goggles, gloves, etc.) Dependant on process Involved.
• Operating faulty plant & equipment Dependant on plant Involved & risk of harm.
• Working without the correct permit Yellow card for 1st failure unless confined space
• Leaving fencing open during work Yellow card for 1st failure
• Using nail guns without the correct eye protection Yellow card for 1st failure
