Zip rigger sign-off

  • Is this instructor signed off to rig zip lines for participants on their own?

  • Please note what needs to be done to get signed off such as: needs more training or needs more monitoring.

  • I am of sound mind and body at the time of this audit and I confirm all my above observations.
