SECOND: GET INFORMATION FROM THE GUEST ON WHAT HAPPENED.

  • If you choose, you can have EMPLOYEE witnesses write their statements at the end of this report rather than on paper. The questions for employee witnesses are after you complete and sign the form.

SUPPORTING DOCUMENTATION

  • Photograph the server chart that shows which servers were working which stations. MAKE SURE IT IS A CLEAR PHOTO.

  • Photograph the guest's ticket. MAKE SURE IT IS A CLEAR PHOTO.

  • Draw a diagram to illustrate what occurred, or where it occured, or anything else that would be helpful.

  • Photos often help someone who was not there better understand what occured. For example, a photo of the area. Or a photo of an item. Note: if you are photographing a puddle, piece of glass or other item please place something next to it for context. For example put a coin or a pen next to it in the photograph.
