General Saftey Observations

  • Tool box talk had been conducted (weekly) INSERT topic in coments

  • Adequate supply of clean water and soap for washing (waterless soap is adequate)

  • Eyewash station, botles, or other designated source is on-site and readily availble.

  • One Person CPR/First Aid trained is on site at ALL times?<br>

  • Emergency numbers are posted and crew members have the means to summon help in case of emergency. Refer to CSE permit.

  • Is housekeeping (truck and site) neat and in order?

  • Are equipment and tools neat and organized?

  • Crew completed daily checklist?<br>

PPE and Personnel Hazards

  • The minimal PPE required is being worn by all (steel-toed boots, reflective vest, safety glasses, 3" sleeved T-shirt, jeans, hard hat)

  • Employees are NOT wearing inappropriate items such as shorts, baggy shirts, loose jewelry. Additionally, long hair is pulled back and kept out of the way.

  • Dust Mask are used where required (Especially Lining Crews)

  • If a respirator is being used in the appropriate environment. It is only being worn by employees that have been fit tested and are clean shaven.

  • Are Subcontractor(s) adhering to ABNA's safety policy?

  • Are the proper tool(s) being utilized for the task?

  • All tools are in good working condition with guards in place?

  • Tools are being lowered by means of rope and not by a power cord or other unsafe means?

Confined Space Entry

  • Appropriate work permits are posted and in use at manhole or trench?

  • Qualified attendant is present during entry?

  • Entrants are wearing body harness and attached to fall restraint device (tripod)?

  • Serviceable tripod is available for use?

  • Gas meters on site have been calibrated on 60 day cycles. Additionally meters have calibration stickers on front.

  • Two (2) reliable forms of communication are being used and are available?

  • Plugging is installed, secured, and monitored with working pressure gauge?

  • Open manholes or trenches are protected (barricades, signs, cones, etc.)?

  • Smoking is prohibited on ABNA property and job sites. There is no smoking on site or in ABNA vehicles?

Hazards Communication, Chemical and Fuel Safety

  • Safety Data Sheets are readily available for all chemicals/cleaners on site?

  • Chemicals are stored in approved containers and are properly labeled?

  • Employees are aware of applicable chemical/physical/exposure hazards, and protective means?

  • Gasoline, Fuel and other flammable chemicals are in automatic closure containers with flame arrestor?
